When did he have you start cycling? I have Hakky as one of the physicians on my list.

I think he had me wait to start inflating a little longer than what Tatem did. If you look through my log it will say but I don't recall off the top of my head.

Also how did you go from 21cm to 26 cm? Were you originally undersized and were you happy with Hakky?

Tatem undersized me which cause me to have to get the revision. I don't really blame him that much, it was more of a result of an infrapubic surgery rather than a penoscrotal (which Hakky did).

I was very happy with Hakky. He is top notch. I have not kept up to date in the past 2 years on the whos who of inplants, but I would only go with: Hakky, Eid, Kramer, Karpman, or the guy in Houston (Clavell?)

Why did Hakky talk to you about doing penoscrotal for the revision when you originally had infrapubic install? My thought was Hakky has been doing a majority penoscrotal now.

I've always had suspicion that penoscrotal may provide slightly better sizing especially for those with peyronie's. I had something similar - infrapubic install by dr goldstein and then penoscrotal revision by dr patel. I gained a bit with the revision surgery but i think it was related to the other procedures done at the same time for my curve. I feel like i didnt ask the right questions especially prior to the surgery with goldstein.

Patel left me almost 70% inflated after revision, since he used the same space for the reservoir from my prior surgery. Usually he leaves about 30-50%.
62 year old, ED+PD, Coloplast Titan 22 cm no RTE in 2019 with Dr. Irwin Goldstein => failure, now with plaque excision/tunical expansion to Coloplast Titan 26 no RTE in 2022 by Dr. Darshan Patel, now with classic pump 2024 :D

Hakky said he tends to do more PS than IP installs. I have seen research that showed PS resulted in more length than IP.

Also, for my specific anatomy he had to do PS, here is a copy/paste from my log that explains it...
[i]
Hakky believes that Tatem shorted me for 2 reasons.
(1) The opening of my urethra is not at the very tips of my glans, its almost to the underside. Basically when I piss, it comes out the bottom of my "slit" and not the tip of it. Hakky said that most implanters bend the glans down when they clasp the urethra for safety. Doing this for me, due to the nature of my urethra, does not allow dilation all the way into my glans. What Hakky did (or does) when he grasps the urethra is he bends it up instead of down. This allows him to see get dilation all the way up without injuring the urethra.
(2) Tatem does primarily infrapubic IPPs, whereas Hakky does peno-scrotal. Peno-scrotal is well documented to allow maximum implantation compared to IPP. Hakky said by doing PS he was able to see first hand view all the way up along the urethra, wheras Tatem's view would have been blinded by the nature that he was going in from the top (infrapubic) and not the bottom (peno-scrotal).

Hakky is an amazing surgeon and I am glad I used him.

Here is an example of the the urethra "stricture"/hyopspadosis that I have. Notice the opening of the urethra is low and not at the very tip.
