I just came from Dr. Kansas' office. I am so grateful that he agreed to see me on a short notice.
What he said was that through his entire career he had never seen anything like my case. He tried everything he could to no success.
Although he managed to slightly reduce the pressure in the failed cylinder which moved me out of the risk zone.
He suggested 2 options - either try to fix the pump or to replace the entire assembly. Given that my installation is close to its expected end of life, and given that an incision would be required regardless, I chose to go with a replacement.
Although Dr. Kansas said that typically for revisions he chooses CX. He claims that I should experience no shortage. Before op and after op my full length was 187mm (7 1/4").

Somebody on the forum mentioned that Dr. Kansas gives preference to longer RTE "for better rigidity". I asked hit that very question to what he replied that normally he preffers to keep RTE as short as possible and only when circumstances require that he would go with longer RTEs.

Now, when the risk of tissue damage has been eliminated, all I have to do is to wait for the insurance pre-approval then schedule the surgery day. Not sure if my insurace would cover it though.
He offered me two methods - pubic and scrotum. My original implant was installed via a scrotum incision and I still remember the recovery pain. Not sure if I should try the pubic incision method instead; but then there is a non-zero chance of nerve damage.

In overall, I was very pleased from my interaction with Dr. Kansas. My provider back in Canada highly recommended him.
What really helped was that during my panic call to his office I happened to talk to a male receptionist who immediately understood the severity of my case and took an effort to make sure sure the doctor would see me ASAP.

When I called to another doctor's office, I was talking to a female receptionist who was just mechanically following the script: "There are no appointments today. And if this is an emergency then I must go to ER."
That was rather frustrating especially given the fact that even if I go to an ER still they would have to page the same doctor as they do not have expertise in deflating failed implants. Just my bill would be unnecessaryly high.

That's great news Danny, and must be a relief. Now you have a plan, fingers crossed for your insurance cover.
There are pros and cons for both surgical approaches. I had penoscrotal and have full sensation back, orgasm 100% of the time from PIV or any other sexual activity. Yes the recovery was longer but I don't know if I'd go infrapubic. I didn't have a choice anyway as my surgeon only does penoscrotal.
72, married, Sydney Oz. PC/nerve sparing RRP Mar 2022 caused 100% total ED. Tried pills, Trimix inj, focal shockwave, VED. All failed. Implanted Mar 6 2025 AMS 700 LGX 21cm x 12mm, no RTEs, MS pump, Penoscrotal. Got back to 6.5" BPEL @ 9m. Loving it.

I mentioned earlier that Dr Kansas was very good. He did mine in '24 and I am still very happy with it. He did it "Infrapubic", and I had very minor to moderate pain with recovery. I didn't use any major pain pills.
I am very pleased with Dr Kansas. His office is very close.

So far so good.
