No much information on Malleable Implants, need Help.

Postby Maxopt7 » Mon Oct 12, 2026 2:50 pm

Hi,

I am 34, facing ED from 4 years. All blood parameters are normal, except cholestrol slightly higher.
My earlier length and girth were 6 inch NBPL and 5.3 inch respectively. but now it is 5x4.5 inch.
I do get erection, even with thought alone, but it is not super hard at all, kind of grade 3. and it wont sustain.
Once I get erect, it goes soft within 5 seconds of stopping stimulation. Even with Tadalafil and sildenafil, it starts to go softer or semi rigid within 10-20 seconds of stopping stimulation. But as long as the stimulation goes on, it stays bit hard, not fully rigid though.
Did Penile doppler scan, had to stimulate myself to get erection even with injection. Mild venous leakage, and grade 3 erection noted.
No problem with blood flow into penis. and also on both left and right side, at the base, there is 0.4x0.5 cm fibrosis, and there is 10 degree tilt to left side. I am practicing VED pumping daily once, from 2 weeks. I reach 15.8 cm length inside pump at 5-7 pressure.
I am considering Malleable penile implant, as insurance does not cover here. But no much information about Malleable.
Do I lose length or girth? How will be hardness for woman on top position? and will I ever regain my previous size with VED? Thank you.

you might lose some length but probably not much maybe one quater or one half of an inch at most is my guess . you should ask the doctor who you choose for the implant what he determines the outcome of length will be as he has expereince .
i lost to much girth since i had more than average [over 6"] to begin with and the rods in my case were undersized at 13 mm . that is the largest available with tactra . if i had gone with rigicon it most likey have beem 14 mm or if the indian shah mybe 15mm . but i do gain girth with engorgment . although thats kind of a catch -22 for a person who is getting an implant since the reason for it is either not getting engorged or keeping it engorged . so the largest size rods are needed as a starting point .
i think in your case the available rod diameter will more appropriately sized
it is had to find onfo on malleable implants and youtube vids are mostly just about succesful operations . you have to contact a doctors office you are interested in having the implant done with and get answers from them . even then i might be like pulling teeth to get a straight answer . they want to make a sale so will shy away from anything i the negitve .....possibly
what country are you planning on having the implant done in ?
American , retired in the philippines .
tactra malleable 13 mm ,in new delhi India . march 2024

You hit the bull’s eye with the girth question for MPP and this shows you have already done nice research.

Some of us may experience corporal smooth muscle atrophy around the static cylinders due to ED progression or diabetes or whatever, and for this reason some of us may lose aroused girth.

I heard one surgeon say that it’s ED’s fault and not malleable’s fault. He mainly implants malleables. I think he made a great point. The rods will remain the same (static and non-expanding diameter), but what’s around the static rod can change as we age.

Good mental arousal and good residual engorgement can enhance the experience. Those with decent residual engorgement seem to be more satisfied.

For optimal outcomes and sizing/placement expertise, try going to a surgeon who does malleables in high volumes.

Thank you for your answer, I am from India, and will be getting it done in India only. So which girth I should take reference? My preED length 6 inch and girth 5.3 inch., currently my size is 5x4.5 inch. So will I lose half inch length from 5 inch? or 6 inch? and if I at all dont lose girth, will I be 4.5 or 5.3 inch? I heard Indian Shah implant is not that rigid. How is rigidity with Malleable as compared to pre ED rigidity? I have forgotten how grade 4 erection feels or looks like.

So, in IPP, residual erection is not required? even without arousal they get full rigid and max girth? and wont the tissue atrophy in IPP? or it does not matter even if it atrophy?

Corporal atrophy can occur with any implant. You could theoretically argue that MPP may contribute to greater atrophy from continuous pressure on the surrounding corporal tissue. I’m not too hung up on that.

MPPs have a fixed max diameter (13mm usually). IPP cylinders fill the corpora fully, stretch and expand the tunica more circumferentially when max inflated (if anatomy allows) and thus compensate for the circumference lost.

These things also vary between individuals. In my case, I lost approximately 0.5 inches of girth over two years with an MPP and regained that back after switching to an IPP. When I switch back to MPP, I might lose approximately that same 0.5 inches of girth again. I’ll be okay with that.

But I heard, we lose size because of ED, not from implant, you mean, if I get a malleable implant, and want to keep it for 20 years, do you mean I keep losing my size continuously? I heard it is best to get implant asap, not to lose size because of ED. this is contradictory right? I am bit scared now. So, either way (with or without implant), I am destined to keep losing my size?

….Losing my size continuously?

After a couple of years, things should stabilize once the fibrous capsule/scar matures. My doctor didn’t indicate to me that I will keep losing size continuously.

These implants are FDA-approved. They may have some risks, as with any medical intervention, but they are the best solution for ED.

You lose size because of ED, among other things. Google "how people lose penile size". That should tell you more about it than I suspect any of us can.

You can also lose size following implantation but from what I've seen it's highly unlikely. In fact greater majority gain either girth or length depending on which implant they pick and how much work they put into it.

If you pick a malleable, you might lose a bit of girth particularly if you're already somewhat girthy. If you pick inflatable you shouldn't lose girth unless you pick LGX whose girth expansion capabilities are limited and you're already girthy than there is a chance.

Dude, there are no guarantees. You always play the odds here with anything. Without an implant your odds of continuing losing size are high. From everything I've seen and read here, on FB, and on Reddit I personally believe that the odds of you losing size following implantation are minimal, but please don't quote on that. It's "forever a rule of thumb" - shit happens.
65 yrs old.
ED since about 2000.
Just moved to Trimix from Edex few months ago.
Implant doctor shopping now but doing BPH first.

If my current girth is 4.5 inch, and before ED, it was 5.3 inch girth, then how much I might lose? Is there any general rule of thumb? I am going with Malleable only, because insurance does not cover implants here.
