I am hoping a few of you here maybe able to help me with some info
First of all I am thankful for everyone that post here and offers advice, I have learned so much from reading on here.
Let me give you a bit back ground on my situation.
When I was 16 I had a serious curve in my penis so I went in to get it repaired (not peyrone but a horizontal congenial penis curvature). The first surgery went well and I seems to heal fast and then went back to sports and didn't realize there was internal stitching and that tore again. So I has to go in for a 2nd time. This time about 12 hours after the surgery there was an issue with a priapism so the surgeon attempted to release it using some procedure in the treatment room to bring it back down. 3 days pass after this and when the swelling comes down they discover that there has been a priapism there for about 3 days, and potential scaring by sludge blood to the corporeal tissue. At that point I refused the previous surgeon touching me and had a winters procedure done right away by another surgeon – this was on my 17th birthday. Fast forward about 6 months after various tests and a venogram and its determined that that it’s all scared down. Giving me implant #1 before my 18th b-day (they had to remove the scar tissue and line the area with gortex first before the implant went in.)
Now 3 failed (2000,2007,2015) devices over 25 years and now I am going for #4 in a few months. My previous device was an AMS 700 CX and this time he is a going to put in the LGX. I am wondering what I should be doing pre/post op to maximize the new LGX going in? I currently still have the CX that is not working (leak) so I am not sure if that would have any affect on maybe using a ved pre surgery date. What are the post op rehab people are doing to maximize the potential of the LGX device?

With all the experience you have had with implants, it should be you offering advice
I have the LGX and as far as I can tell the post operative stuff is basically the same as the other IPP's. When you get the go ahead start cycling, much as you did with your previous implants. Some docs suggest a pre-operative regimen of pumping with a VED, not specifically for the LGX though, and some patients feel it helped them maximize implanted size. My doc did not suggest it.
At seven months I cycle my LGX about 5 times a week, sometimes for use, sometimes to continue the cycling routine. I often inflate at bedtime and it stays inflated for a good part of the night. I will deflate if I wake up in the middle of the night, sometimes not; it depends on how much I had pumped the previous evening. As I said, I was not given any special instructions for the LGX, just the same as for other IPP's. Maybe other docs gave their patients LGX instructions and will join the discussion.

That's an interesting question.
I was told the VED Therapy is important before having an implant as it constantly helps breaking up the scar tissue and maximize your size.
My doc and other guys in here have told the VED is to be used BEFORE the operation, definitely not after.
I believe the VED could damage the implant, however since your device is no longer working maybe you could try that, but I would ask your doctor first.
Maybe your doc could try to use a bigger cylinder when having your prosthesis replaced.
I was told I would probably need a bigger implant when it comes time for a revision.
Taking advantage of your experience with implants, let me ask you a question: Have you notice any difference in your size when having an implanted replaced?

I've had 10 implants myself of just about every make and model. You can use a VED while your failed implant is in place (with doctors permission) but use caution regarding pain. By now, you have a well developed tunica and you're somewhat protected against shrinkage because of that and due to having the CX still in place.
There is no special preparation for replacing the CX with an LGX, I did exactly that too. I did use my VED on a daily basis. I believe and my doctor (Douglass Milam) said he believes use of the VED was beneficial. Just go light to moderate on the level of vacuum pressure you use.
I'm much happier with my LGX than I ever was with the CX. It works very well and has given me more usable length than the CX ever did. I can't say that this is because of the implant. I believe it's more properly sized by Dr. Milam than previous doctors. It does however seem to be expanding slowly.
You won't (or shouldn't) see any shrinkage but if you do, just remember that the LGX will expand in both length/girth and over time it MAY help restore any losses.
I think the VED may be helpful as already stated, for reasons other than size. It forces blood flow and keeps tissues healthy which is a good thing prior to surgery. That said, I'm no doctor and you should ask him. All of us have different physiology and anatomy issues, and your's could be unique given your prior history. There could be factors connected to your earlier conditions that have a negative bearing on this, but for me it was the way to go.
Good luck and enjoy your new LGX when the time comes. So far it's the best device I've ever had.
P.S. My wife is thrilled with the LGX. She said it's WAY better for her than any of my previous implants. She's even happier about it than I am, and that was. My goal.

The docs who were willing to see me and discuss an implant said the LGX is the preferred alternative for a replacement now and almost always a replacement goes up a size. I do not know if the GorTex will keep you from going up a size but other than that I think you have nothing to do to prepare for the surgery. Man that sucks you went through so much as such a young age. I think it is awesome that you got an implant early instead of waiting years like many of us. I had ED over 14 years before any doc mentioned an implant nor did I even knew there was such a thing. 19 years later in Feb I finally get one. Good luck. I am sure it will be awesome! My wife mentioned yesterday something about we have gone without most of the time for 20 years because of this and she is glad she did not marry me for the sex 32 years ago.Best of luck! d

Thanks for all the advice, I am scheduled for surgery on Feb 24th so the wait is not that long.
I will call my doc and see if pre-op VED therapy can be OK'd while I have the existing failed CX in place. I believe that the gortex was removed and there is just the AMS 700 CX implant according to the post op report my family doc gave me to read recently. It was tough going through this at such a young age but with the tissue being all scared down I was only left with the implant option at 17. I actually had to fly out to a different city because there wasn't a urologist in the city that had done an implant on anyone as young as me back in the late 80's. Sometimes it was hard (no pun intended) dating but I never got rejected due to having bionics.
I am actually looking forward to getting some size (length) back with the LGX, luckily I was above average to begin with so the loss in length due the implants only hurt my ego a bit. I can now work at maybe reversing that hopefully.

Gortex can't be removed once installed w/o serious damage. I have a lot of gortex in my body. Doc put in more in 2009 after telling my wife and I at 2 presurgery visits that he would never use it because revisions were impossible , but he used gortex anyway to " get the surgery over quickly". Saw another doc a month later who said it needed to come out as the outcome was less than expected and he called Gore company in Texas while I was in the office and they told him after a month it cannot be removed with out destroying all the surrounding tissues because the tissue grows through the screen mesh of the gortex. They told him it would take a major open surgery to try to disect the gortex out of the tissue that surrounds it. Overall the LGX still seems the top choice for revisions and docs have told me they usually can go up a size on the revision. Good luck. d

You are correct that it was not removed, but incised through. 21cm LGX would be the step for me, are there concerns about maintaining rigidity with the larger LGX? I have never had glans problems it was just the corporeal that was messed up for me.

I was told that the Titan is more rigid, but it makes no difference because both brands pump up more than the hardness you need. Docs told me LGX can pump up harder than you need if you pump up fully and in most cases you would use few pumps for intercourse than stretching your rod. Main difference in Titan and AMS/ Boston Scientific was the Titan will not deflate or be as flexible when soft and more prone to kinking in the tubes. d
