all of what you mentioned are possibilities. Its extremely hard but you don't have to pump that much either. The tips can be felt in the glans by hand so I'm guessing she could feel it with her Mouth. I don't have first hand experience lol but if you have had oral with a few natural hardons I think you would be able to tell something was different.
