1. Do these things ever have leaks? If so, does it require surgery to fix?

2. I'll be good for maybe 10 more years. Do they ever wear out, during this period? What do you do if this occurs? More surgery?

3. Do you ever have unintenended "deflation" during intercourse?

rbig5000, I had all the same worries and then some. I've found the following, in answer to your questions in order.
1. Rarely. Yes, if they should leak surgery would be required. My Doc said any "action" is OK except no needles!!!!!!!!
2. Again rarely the failure rate is very low. If so of course surgery would be required to remove, if so. See on this site a few guys reported failures.
3. NO. Deflation has to be a very deliberate action, at least with a Titan OTR as I have.
I feel you are worried about the wrong things. One of the higher rates of things to go wrong is a !% chance of infection. Make sure you have a Urologist that's done at least a few hundred of these implants. My biggest complaint is the difficulty of operating the darn thing to get a decent erection. You really have to crush the ball in your scrotum. They tell me, within this forum, this gets easier. Good Luck, Tom

Yes it is. That's why they call this section "The Final Frontier" I feel it could also be called " The Court of Last Resort". The Docs try all avenues of correction of the ED before going to the implant. I think most guys know when they tried everything that an implant is the only way to go. Regaining something you enjoyed so much and was so gratifing between you and your partner, some say regaining their manhood. I choose to regain what I had after considering all the risks involved. Rbig,

I developed ED in 2004 after an Open Radical Prostatectomy and the "standard" ED Treatment Modalities did not work
satisfactotily for me.
Finally, in December 2007 I was implanted with an IPP (Inflatable Penile Prosthesis) and, thankfully, this corrected my ED.
Like Tom, I have the Coloplast Titan Implant and have had no problem with it for over 4 years now.
As with any mechanical device, there is always at least a small chance of a malfunction.
Should this occur, you would have to undergo a surgical revision to correct the problem and must accept that possibility going into the Implant Procedure. In retrospect, I would go ahead with my implant again in a heart-beat.
To answer your question, I have never had "accidental deflatation" during intercourse and have never heard of this
happening. One very important point --- the final decision is yours and yours alone!!!
