Greetings to the brotherhood!
So, found this site roughly 3 years ago and probably read every thread on here. Multiple times. I'll give a little intro before delving into my requested advice wisdom.
31 y/o, significant ED since my late teens, managed well with pills. At first, they worked like clockwork, but 3 years ago, I realized these things often don't last forever. I have managed to live a fairly normal and productive life. No kids, but a stunning fiance and a great career. I accepted 3 years ago that an implant was more than likely inevitable, however, this didn't stop me from being absolutely terrified for a multitude of known reasons listed frequently throughout this site.
In short, the pills still work. Most of the time. That being said, I am exhausted. Mentally for thinking about this issue every day of my waking life, physically for the stress I am putting on my body, and emotionally for desiring a simpler way of life; a life void of thinking about how your dick isn't normal (not that a robo-dick is normal, but hey, I haven't had a good bulge in a while so bring it on).
I will spare everyone the "what surgeon, what prosthesis, will I lose length, what will my spouse think," type questions. I have done my research, money is really not a factor when it comes to the rest of my life, and I am aware of the many risks and rewards of an implant. While I value the many 50, 60, and 70 y/o's constantly claiming this is the best thing you can do for yourself, or "what do you have to lose? just do it," I am seeking a few specific insights to a couple of questions. Forgive me if they sound odd; they are really my biggest concerns.
1) I have a great dick when aroused. Roughly 6.5", over 5" girth. The catch? My flaccid resembles a skinny, short gummy worm.
2) Glans engorgement
Further explaining these concerns, I am deathly afraid that d/t my flaccid size, a stretch test will not do justice. While there are times the stretch matches my erect length, and others when it is sizably restricted (ligament issues? temp?). Is an injection the only way for a surgeon to know my erect length, or will the anesthesia magically relax my dick enough for an appropritate measurement? Since ED, I have been a grower, but as a shower, obviously I want to be showing more than 5". From the start I have favored Titans (no offense to AMS implantees) but fear one look at my flaccid will result in a smaller, AMS model. I know some length may very well be compromised. I can't lose the girth.
Second, the topic of glans engorgement. I know this has been covered, but some fresh comments would be approciated. I have a tiny flaccid glans and a large mushroom when erect. I understand this isn't guaranteed, and to be honest, I'm not really expecting this to magically come about after an implant in my personal case. I don't find it ideal to keep downing Viagra once you are implanted. Is sex still enjoyable with a non-engorged glans? A little worried about insertion and overall erosion with a non-engorged glans. Painful?! I guess I want to know if this is a major dealbreaker, preferably from those who don't really have much.
On a side note, has anyone become a father with an implant? I would imagine it's still possible, but never see anyone posting about it.
Well gentleman, I want to thank all of you for reading this, and a special thanks to those that reply. By no means am I discouraging anyone over 50 to respond. If you have some insight, please share. Especially if any of you are/were in the same boat. Young members, feel free to reply with any encouraging thoughts/advice. In essence, I am in a fragile state (not dangerously fragile), wanting more than anything to start living my life without thinking about my dick. This website has been a source of hope for me over the years and I am so thankful to have a forum to ask questions with this level of intimacy without seeing a shrink or a uro.

I'm well over 50, but I'd like to reassure you that the surgeon's stretch test has no bearing on the size of the implant that is installed. The only test that counts is the internal measurement that is made after you are out on the table and the initial incision has been made.
Dave, 80, Maryland - Implant (Titan) 2008 by Dr. Andrew Kramer (failed Sept 2020) - never used due to a stroke that, among other things, ended my sex life.

The stretch test prior to the surgery gives the surgeon the basis for assuring you what he can give you in the way of size. He knows that if he can stretch your penis X length then he can put an implant in that will, in fact, do the same thing. My surgeon assured me I would be a minimum of slightly over 6 1/4" and seven weeks out I am 6 1/2". He also was pretty certain that with a Titan I would take a 22 cm implant and I did. My glans does NOT engorge without meds or a VED and sex are GREAT. Of course, I am an old man and maybe I don't know what sex is supposed to feel like. Sex has been my preoccupation for 55 years and I could usually have multiple orgasms from one erection without a break anytime prior to prostate surgery.
Good luck. This always offends people. Many people disagree with several of these. You will have to decide what your standards are and the degree of risk you are willing to take.
ut my advice is to pick a surgeon:

2. that does not need or want a company rep in the operating room with him.
3. does over 100 implants a year.
4. implants bot AMS and Coloplast
5. takes several implants in the OR with him
6. Has actually removed an implant because he thought he could get better results with another implant.
7. Tries to avoid the use of RTE's

Thank you both for your responses. I've seen some Kramer vids of "smaller" preimplanted patients and they wind up getting some decently lengthed prostheses. You guys give me hope.

I am 44 years old. When I started with Peyronie’s disease at 40, my first Uro told me I should think about an implant. MAN do I wish I took his advise! It probably would have saved me around 2 inches of length. But- I wasn’t ready. I thought implants were only for older gentleman, not me. Now, I am 44 years old, I got implanted 9/6 and I can honestly speak to you about the psychological piece you are asking about. I know that I haven’t been activated yet and haven’t used it yet, but since I was put on the docket to have the surgery till now, I have been at peace. Suicidal thoughts, anxiety, depression all disappeared. Because I know I got a quality product. When I was toying with the idea of getting implanted, my wife told me” it can’t get any worse!” Meaning the sex was aweful. This was the impetus for me to go ahead with the surgery. She has been supportive in my healing- pretty much making sure I am well -like any other surgery. But, I think we are both waiting on anticipation to see if the damn thing works! I assure you- this is good waiting not bad waitIng.
As far as engorgement, my glans gets engorged and actually I get a minor chubby. Especially in the morning. Also, regarding children, I am actually going to get a vasectomy in 3 months to prevent any accidents!

look up "Broken Boner Radio" hosted by Danial Cainfield. He mirrors your life and has worked through it. ED since teens, Got married and raised a family. He also works with San Diego Sexual medicine and Dr Goldstein. Great group. Join Danials group and he will answer all your questions. He helped me and is a personal friend of my family.
On glans. you may need to use pills to engorge the glans. The implant does nothing for glans. If I could, I would use pills to engorge my glans but my nerves are all removed.

I'm in my fifties and it has been almost a year since I was implanted. My implant has become a part of me and I don't really give it that much of thought anymore. Since you are a grower like me, you will probably love being a shower after getting the implant. My glans engorge just like before as that engorgement comes from a different blood supply than the blood flow for an erection.
Now obviously having an erection anytime and for any duration will do wonders for ones sex life, but I must say that the impact psychologically should not be overlooked. After the pills stop working years ago, I became obsessed with trying to find a "cure" for my ED. I can't even begin say how many different supplements and combinations of supplements I tried. I would spend hours surfing the internet to find a magic bullet. I did try injections which initially worked, but also became unreliable and painfull.
Now post implant, a great weight psychologically has been lifted from me. I don't have to think or worry about ED. Little things like watching a movie where a couple starts getting into a love scene don't upset me or make me feel inadequate anymore. You really can't put a price on that at any age.

I want to welcome you to Frank talk. I also am a newbie . I have ED for over 10 years However: I am currently waiting for scheduling for implant surgery. I wish you all the best and praying that all will go well for you.
I am 62 years old. Full blown Ed for 10 years. married for 18 years. my urologist dr. Kershen. Implanted October 15,2018. AMS 700 LGX 18cm length by 12 mm diameter I live in Coleman, Mi. USA : )

Glove...far be it from me to be an 'expert' on implants but I would like to weigh in on the peripherals of this site and the procedure. You are doing your homework: researching, asking questions, and (like me) losing a lot of sleep and living with the anxiety of the operation and ALL its potential up and down sides. At 71, I truly questioned whether I wanted to be implanted so I greatly appreciated lurking FT for a while to gain some perspective. Everyone IS built differently, reacts to pain differently, heals differently, and deals with stress differently. Take what advice you receive here with as much salt as you want since 'your flavor' will not be exactly the same as others. The more I got into the details of it all, the more terrified I became and had to take a break from reading all the horror stories posted here. My original uro surgeon from Virginia Urology fired me when I started asking too many questions and contacted his Coloplast rep for product details BEFORE my scheduled surgery. I'm not sure how some guys do it but I certainly got my 'comeuppance' and am glad I learned his demeanor before going under his knife.
You are the master of your own body and what happens to it. With your partner's support, a compassionate and skilled surgeon, and the wise counsel of FT posters, you will come through an implant surgery and follow-on recovery well and, finally, put ED in your rear view mirror. What I have learned through the very brave souls here is that an IPP is a process that takes time to complete (if that really ever happens). Learn everything you can about it and then just do it with a positive outlook, a smile (once you get past the pain), and the knowledge that others on FT WILL have your back and encourage you to persevere! I have one of those guys very close to my upcoming surgery location and he is fantastic in the way he continues to help me over the mental hurdles in a way that I cannot do on my own.

77; ED at 50. Fired by 1st doc (Szobota - VA Uro) too many q's & contact w/ Coloplast rep. New doc: Ellen (VA Uro) implanted 11/8/18. 22cm Titan + 2cm RTEs; moron docs, product rep, intake/ dischg nurses! NEVER again! L- 6.75"; G- 5.5" oval.

Whatever blood flow you have now, you should retain after implant as long as the surgery is done right. You should also retain your size. I was implanted at 35 and I’m happy. Age has nothing to do with it. If you need it you need it.
