Those number have changed a LOT...BCBS of Georgia and Cigna both now have "hard exclusions" on their policy documents stating they exclude any procedure for sexual dysfunction. Cigna's even says "(Including IPPs)" in the document. At least that's for the policies I have access to...I'm sure if you paid more in policy premiums they might cover more.
58yo in good shape looking at Coloplast Titan. Tri-Mix still works but is a drag.

When I was first researching everything, I looked through the details of my insurance policy (Anthem Bronze Plan - High Deductible, Self Employed in California) and it had these "hard exclusions" for sexual dysfunction. So, I fully expected to go out of pocket for it and even Dr Clavell informed me that my insurance wouldn't approve it.

Yet, my eventual surgeon, Dr Karpman, got it approved 5 days after my initial consult. I was shocked, and happy of course, and I don't understand why but I'm not going to try to answer that!
