Hey JH, sorry for not responding sooner, I got hit with some heavy depression and didn't feel like talking at all.
To finally answer your questions, I do still get some engorgement some of the time, but not nearly enough. With the help of a cock strap the blood is trapped in there much better and my head gets really big and my dick rounds out. It has been the only way I have been able to jerk off so far. They are placed about half way up the glans so there is some give or softness when my glans are not engorged, but it does the job.
Not perfect but I can live with it.

What I found works the best is a leather cock band with buttons and snaps that goes over the top of the base of the cock and under the balls, cinched tight, it gave me the best erections before and that is still the case with an implant.
I plan on trying other bands as well with ball separaters and also ball stretchers. And whatever the hell else is out there.
Depending on how much things improve naturally I may even consider any surgical options available.

I have suggested to him to try the Osbon tension bands. I use two of them together and they overcome my venous leak. They are the only really good bands I have found.
Problem is 15 mins max time. That's fine if it allows a good erection.

How tight is the ring you use?

But I think it's more the blood supply going along the underside of the shaft and balls which is why the band works for me where the tension rings do not ( because they only go around the shaft).
I was experimenting yesterday to see how I could get the best glans engorgement. Grasping the base of my cock barely increased engorgement but grasping my cock and balls at the same time did significantly increase engorgement. Also simply pulling down on my sack seemed to work too which I thought was odd.

My surgeon has no problem with my use of the band and I experience no pain or discomfort from using it.
I keep it tight but not so tight where it starts to cuts off circulation.
