Bear5000 - Help regaining usable length.

Bear5000 wrote:Thank you for your discussion of penile length. Perhaps you can comment on my situation. I had a coloplast titan implant May 28 this year. I have had ED for some time and I am 80. I am also on several prescription drugs such as metoprolol, pradaxa, azor, rosuvastin, spirolactone, and furosemide. most of which contain beta-blockers. One day before the operation I had a 5.75 inch erect penis as shown in the cylinder of a vacumn pump and a 5.6 inch erect penis as measured after injection. Four months now after the operation I have only a 2.75 inch erect penis. Somewhere in the process I lost 3 inches, and a 2 inch dick is not worth much in bed. Any advice you might give me would be appreciated on ways to correct the problem and get back an inch or two.

I hope this does not violate forum rules, but I took the liberty of posting this here where it could have its own topic and bear could get some help.

Bear, first off, you are in the right place. We would like to know a lot more about your situation. Like Larry, our moderator suggested, If you can go to your profile and make a signature line it would help. If you can't figure out how, we will help.

It is very unusual for someone to lose that kind of length.
1. Tell us more about your surgeon,
2. have you discussed this with the surgeon?
3. Can you feel the tips of your implant in the head of your penis?
4. Do you know what size implant you have?

If you would like to talk by phone send a personal message to me and we can do so.
Good Luck.

Something is not right. Never heard of anybody losing that much in length. Years ago when they first started with implants it was common to lose one inch, but now most surgeons have it perfected to where you do not lose any length.
I would go back to the surgeon and find out what is going on. Pretty sure something can be done, if not by the original surgeon, might check with another.

Thank you kindly for your response. I will try to answer your questions below.

1 & 2, Yes. I provided surgeon with photographs of the erect penis showing 5.75 inches before surgery and he said that I should expect close to the erect length with the titan implant. After surgery, he was very supportive but never made a serious effort to solve the problem and finally said that he could not help me with the problem. I asked him to x-ray the implant to see if anything was out of the ordinary but he did not want to. Now I am pretty much on my own after 4 months.

2. The length of the implant was 17 cm.

3. Yes, I can feel the tip near head of the penis.

4. There has always been a good deal of pain especially in the form of what seemed to be needles pricking me from the inside and it seemed like the implant was punching me. Very uncomfortable and required some form of pain killer so I can focus my attention on my work.

Everyone is different and the length of the chambers that is internal vary, however, I was 6 1/4" for the first weeks after surgery and now 6 1/2" and I got a 22cm Titan with no rear tip extenders. That is a 5 cm or a 2" longer implant.

Next, the tip should extend into the glans (head) 1/3 to 1/2 of the length of the glans. So the tips should go close to half-way in.

How long was your implant deflated after surgery before you began to inflate it (referred to cycling the implant)?

Do you cycle the implant now? How often do you cycle? How long do you leave it inflated? Do you pump until you cannot squeeze the ball anymore?

When fully inflated does the implant feel like it is stretching your penis (discomfort)? Is it lose or floppy or very hard?

Do you have any idea of how many implants your surgeon does a year? Did he even try to explain or give a reason for your loss?

I know this is a lot of questions but try to answer all of them so we have an idea of what is going on. Also, the stabbing/sticking pains like a needle sticking you are not uncommon during healing for the first month or two. Usually, it does not require prescription painkillers after the first month, however. Also, the drugs should not affect your implant size in the least after surgery. They could contribute to ED or even some shrinkage before surgery. How close to surgery did you measure and was your measurement accurate?

You don't have a signature but I read from this that you inject. You MAY get some back by pumping a few times a day and leaving it pumped up for 15 to 30 minutes. This is not going to happen overnight, but you may get some length back. :)

Follow up answers:

As far as I know I did not have any rear tip extenders, but it was hard to get information on the details of the implant.

After the surgery, I had a lot of swelling and pain so it was at least three to four weeks before I tried to inflate. Then the penis was so short that after learning to pump I could not see any effect at all on that part of the penis that extended outside my body. It took me nearly 3 months of frequent efforts at pumping to get an obvious effect. Then the erect length was slightly less than 3 inches and the girth did not increase at all. In fact the girth is less than before surgery, but that may be because I can only see the tip of the implant extending out of my body. I had a hard time learning to deflate but finally learned. Now I can inflate and deflate without trouble but still there is nearly no effect in girth and no effect in length. I pump it up until the bulb is hard and impossible to squeeze any more. I think the inflate and deflate process is working right, its just that I have less than 3 inches to work with.

The fully inflated penis is hard, I can feel the inflated cylinders hard and tight. There is not much stretching of the penis though.

I know that my surgeon frequently does implants but I never got an answer to exactly how many annually. His reason is that I am 80 and have been troubled with ED for about 20 years and thinks the penis has just withdrawn inside my body and that there is nothing that can be done. I find that hard to understand because I could easily get 5.75 inches with the vacumn pump or injection up to 2 or 3 days before the surgery and I could squeeze my flacid penis in my hand and it would extend even in that state to about 4 inches. Historically, from my 20s into my 60s I always had 5 or 6 inches to work with and never had any trouble with penetration or performance otherwise. The ED set in as I got older and became more pronounced after I had a stroke and the cardiologist put me on drugs to control blood pressure which contained beta blockers. Now my blood pressure is usually 110/65 but my heart rate runs around 80 because I have arterial fibrillation (AFib).

Many thanks for your comments.

I’m wondering if Dr Eid or Kramer might do a phone consult to problem solve to see if there was an error and you need a revision?

I think they would probably need to see the mess in order to know if they could correct it. That's what happened to me and I had a seven hour drive, with a back injury, to accomplish in order to get my date with the scalpel. :)

Bear, thanks for the background info. This is one reason that I HATE IT when a surgeon does not do a pre-surgical measurement for your medical records. If he had, he would have either acknowledged then that your penis had atrophied over the years and only had a 3" potential or acknowledge that with normal aftercare you could expect 5 3/4". There should have been no surprises. He also should have pumped you up in the OR and assured you were very close to your maximum potential at that point. If you were then but are not there now, it is a sign that fibrosis set in during your long delay in fully cycling.

I can only speculate because 1) I am not a doctor and 2) I cannot even see your penis, but I will give you some info and speculate a little and offer the only suggestions I can come up with.

This is what I expect from a well-implanted penis that has a Titan implant based mostly on my own experience and a little of what others have detailed. This might help you assess whether the implant was big enough for your potential and whether you are properly inflating, and finally, if there is any hope the implant will stretch you. I suspect this will be a long post.

Immediately after TOTALLY deflating my implant my penis can look deformed and flat in spots. That does not last long but I can feel the flat cylinders until I pump up again. The cylinders are flat enough to feel the edges. They also have significant folds in spots like when you flatten a plastic straw and fold it like an accordion. You get the sense your penis is stuffed full of the implant, maybe overstuffed. As I begin to pump the cylinders first round out then the folds begin to come out but I must pump past intercourse sufficient erection almost to the point of maximum inflation before I can feel up and down the shaft and feel no folds or dimples in the cylinders. By the time those folds are gone, I feel tightness in my penis. I think if your folds disappear long before you are totally pumped that it is one indication your implant is undersized. Of course, if you have no folds even when totally deflated that would be an even bigger indication.

When fully inflated you should feel the tips well into the glans. If not or if the glans is floppy it is a possible sign of being undersized.

When I pump the pump ball gets harder to pump at about 25 or 30 pumps and the last few pumps have to be partial pumps. My penis is more rigid at this point than even an 18 year old's. If I wait a few minutes I can force a few more partial pumps. If I stay inflated, within 30 minutes it is getting uncomfortable from the stretch. Leaving it fully inflated for an hour is a challenge. If you do not experience this discomfort it may also indicate you are undersized. It would also indicate that you are not stretching or expanding the penile tissue with full inflation. I think it would be an indicator that you should check with another very skilled specialist if you can travel to find a high volume surgeon.

If you ARE feeling stretch after by 30 minutes or so it indites that the implant is stretching you in either girth or length, or both. That may mean that your long period before total inflation allowed some plaque, fibrosis, or scar tissue to develop and you have to put in some work to stretch it out. You can do this by cycling 2 times a day for an hour. If you cannot stand it for a full hour then deflate a little once or twice during the process just enough to give yourself a break and pump back up. If your penis feels like the implant stretches it you can also use the VED as Larry suggested. For me, it works best if I inflate about halfway, use the VED to engorge (use the band), then inflate to the maximum.

By the way, did he go in through the scrotum or the lower abdomen? AND were you sent home inflated or deflated?

Let us know what you think about your sizing and whether the implant is stretching you after you inflate for an hour.
