Bear, thanks for the background info. This is one reason that I HATE IT when a surgeon does not do a pre-surgical measurement for your medical records. If he had, he would have either acknowledged then that your penis had atrophied over the years and only had a 3" potential or acknowledge that with normal aftercare you could expect 5 3/4". There should have been no surprises. He also should have pumped you up in the OR and assured you were very close to your maximum potential at that point. If you were then but are not there now, it is a sign that fibrosis set in during your long delay in fully cycling.
I can only speculate because 1) I am not a doctor and 2) I cannot even see your penis, but I will give you some info and speculate a little and offer the only suggestions I can come up with.
This is what I expect from a well-implanted penis that has a Titan implant based mostly on my own experience and a little of what others have detailed. This might help you assess whether the implant was big enough for your potential and whether you are properly inflating, and finally, if there is any hope the implant will stretch you. I suspect this will be a long post.
Immediately after TOTALLY deflating my implant my penis can look deformed and flat in spots. That does not last long but I can feel the flat cylinders until I pump up again. The cylinders are flat enough to feel the edges. They also have significant folds in spots like when you flatten a plastic straw and fold it like an accordion. You get the sense your penis is stuffed full of the implant, maybe overstuffed. As I begin to pump the cylinders first round out then the folds begin to come out but I must pump past intercourse sufficient erection almost to the point of maximum inflation before I can feel up and down the shaft and feel no folds or dimples in the cylinders. By the time those folds are gone, I feel tightness in my penis. I think if your folds disappear long before you are totally pumped that it is one indication your implant is undersized. Of course, if you have no folds even when totally deflated that would be an even bigger indication.
When fully inflated you should feel the tips well into the glans. If not or if the glans is floppy it is a possible sign of being undersized.
When I pump the pump ball gets harder to pump at about 25 or 30 pumps and the last few pumps have to be partial pumps. My penis is more rigid at this point than even an 18 year old's. If I wait a few minutes I can force a few more partial pumps. If I stay inflated, within 30 minutes it is getting uncomfortable from the stretch. Leaving it fully inflated for an hour is a challenge. If you do not experience this discomfort it may also indicate you are undersized. It would also indicate that you are not stretching or expanding the penile tissue with full inflation. I think it would be an indicator that you should check with another very skilled specialist if you can travel to find a high volume surgeon.
If you ARE feeling stretch after by 30 minutes or so it indites that the implant is stretching you in either girth or length, or both. That may mean that your long period before total inflation allowed some plaque, fibrosis, or scar tissue to develop and you have to put in some work to stretch it out. You can do this by cycling 2 times a day for an hour. If you cannot stand it for a full hour then deflate a little once or twice during the process just enough to give yourself a break and pump back up. If your penis feels like the implant stretches it you can also use the VED as Larry suggested. For me, it works best if I inflate about halfway, use the VED to engorge (use the band), then inflate to the maximum.
By the way, did he go in through the scrotum or the lower abdomen? AND were you sent home inflated or deflated?
Let us know what you think about your sizing and whether the implant is stretching you after you inflate for an hour.
