As tomas1 pointed out, external measurements are only of your penis in front of your pubic bone, outside of your body. Other variable that CANNOT be measured in an exam office are: Distance from the tips of your tunica albuginea (variations here are minimal and probably on the order of a centimeter or two, so MIGHT be estimable) is one. The really variable one is the depth of your crus, which varies considerably between individuals.
The ONLY measurement that tells a surgeon what size implant to use is the measurement he takes after he cuts open your tunica ans inserts the measuring tool proximally into your crus and distally into your penis tip, bot measurements of the inside of your tunica albuginea. He then adds the two figures together to determine what size implant to use (Montague, et al recommend subtracting a nominal bit from the figure to ensure against erosion through the glans, but this practice is largely eschewed by surgeons experienced in inflatable implants). The implanted device then has absolute determination of your ultimate external penis size. Whatever the surgeon does, the implant, where it eventually sits in your crus determines your ultimate dimensions and angle of erection. The implant has the final say as the surgeon has limited control over the finer details of where the implant "settles" in.
Having said that, I will point out that this is where experience is VERY important. A surgeon who has done hundreds or thousands of implant and (THIS IS IMPORTANT) studied how his results have turned out!!!! will be in a much better position to make the best estimates/predictions of the eventual outcome.
So, he can guess what implant size he will be able to fit into you, but that is only a guess.
