I have a venous leak and peyronies. My tunica doesn't get firm without pd5s currently. Trying to gauge what to expect for firmness and girth after I get my implant.

There is some evidence that up to half of men with venous leakage might retain SOME erectile function (WITHOUT inflating their implants). Not necessarily improvement, but some retention. That means the other half (all the men in the study got tissue-sparing surgery) lost what little they had. ONE man sometimes (without pumping up the implant) got just enough erection for coitus.

This is not something to count on, by any stretch of the imagination.

I have venous leak and got nothing from the implant. In fact, my (already limited and short-lived) ability to get an erection was completely gone after the operation. THAT is the outcome ANY MAN should EXPECT from the operation. Any spontaneous erectile ability (regained, retained or improved) should be considered a miraculous gift.

Sorry, I should have been more specific. He could get rigid enough for coitus (barely) WITHOUT pumping up the implant. WITH pumping up the implant, of course he was fine.

In my defense, my whole post was talking about venous leakage mitigation, not implant operation or effectivenes

The use of a cock ring with the implant will reduce venous leak outflow. Some guys here use the rings with the implant as a result.
I do not know of anyone who noted that inflation of the implant alone was sufficient to make a significant difference to venous outflow reduction.

Credit to Ls for saying it as it is, and not trying to butter up his situation. And I 100% agree with him what he says about what to expect. Anything beyond that, should be considered a bonus.

I would like to add to this topic, that if I remember correctly, my doc told me when discussing with him around the time of surgery, that a large implant can trap blood in the glans and help counteract the effects of venous leak.
Not 100% sure I remember correctly though, this was a few years ago...

In my case, I have been lucky to get great blood flow to my glans and it stays hard during sex. This is irrespectively of how hard I pump. These days I almost always pump as hard as I can, using two hands and full force, when having sex. And my glans still gets hard an my shaft gets warm and "soft" (the skin that is. The cylinders get super hard).
This was not the case always in the beginning though. It took a long time (perhaps 2 years) before I could enjoy the implant pumped to maximum hardness. Before that point, I would have an unnatural, blood-lacking, feeling if pumping too hard.

I am somewhat confused by this discussion. Unless I am greatly mistaken, an implant fixes venous leakage. I have read testimonies from men who had severe VL, either congenitally, from injury, or from unknown cause. I know that it plagued me a few years ago. But the constant take-away is that an implant will correct the condition. If this is correct, what is the speculation for? I must be missing something.

It sounds like you're confusing terms. It doesn't FIX venous leakage. It replaces the erection mechanism that was faulty due to venous leakage. The cylinders create the rigidity that was previously provided by the erectile bodies which may be failing to operate properly due to venous leakage.

The problem is that some men lack erectile function of the spongiosum, the sheath which surrounds your urethra on the underside of the penis, along with the glans, which is a continuous compartmentalised tissue mass. This is not catered to by the implant, which is restricted to the cavernosal erectile bodies.
So does the pressure if the inflated implant in the cavernosa, press upon the veins draining the spongiosum enough that it would reduce outflow, facilitating erection of that spongiosum?

Or does it impede blood flow to the glans? Does it do both?

That's exactly my issue. I was implanted about 50 days ago and I have issues with spongiosum. My dick is flat and glans is soft. When aroused and in constant stimulation I can get some blood in to it but when I stop stimulation it leaks in 2 seconds. Before the implant spongiosum worked pretty well and I had no issues with soft glans, issue was in cavernosal bodies. So for me there spongiosum leak is worse than before. I'm wondering if a titan that has wider cylinders may be better to stop the leak, now I have ams cx.
