Welcome to the forum, 23yopriapism. I sympathize with your painful breakup and hope for a cure less invasive than an implant.
Please do not discount the knowledge that older men might be able to contribute. All elders were young once.
Some additional facts might help with a diagnosis. After your breakup, did your erections (morning and during your waking hours) change?
You described your erections before the troubled relationship very well.
Please describe your erections after the breakup but before taking the Donaren?
Please describe your erections after the Donaren? (I note that Donaran is related to Trazodone, which is better-known in the U.S.A. where most of the members of FrankTalk reside.)
Obviously your erections have changed. Did your libido also change?
The urologists' opinions that your E.D. is psychological is understandable and they might actualy be right given the timing of the breakup and the onset of your E.D. But knowing the exact timing of your E.D. symptoms would be revealing.
As I read your post, it appears that you had priapism one time immediately after taking Donaren. Is that right? Trazodone has that listed as a side effect. Priapism of more than 4 hours can cause permanent damage and the usual advice is to go to an emergency room to relieve the erection after 4-6 hours. Did you make it clear to those urologists that you had 10 hours of priapism?
I do not know what sort of damage can be caused by extended painful priapism or if an ultrasound of a flaccid penis would discover it. More research is needed.
At your age, I truly hope that an implant is not needed. A less invasive way to restore your prior potency would be FAR superior.
By the way, your English is very good. And your logical flow of thinking is excellent and grammar is clear.

i have had 2 times. I always forget that i had one day, for like 4 hours. And then the other day for like 10 hours i guess. Both of them were in the morning, after waking up.
Are you still taking Donaren/Trazodone?
People develop a tolerance to this (and other related drugs) and it can be very difficult to discontinue taking it. (It can be dangerous to stop without supervision by the prescribing doctor.) It appears you have one side effect (priapism) of this drug. Your dislike of this type of drug is, in my opinion, a good thing.
I would DEFINITELY not take any erectile function drugs while taking Donaren. Because you had bouts of priapsm, I think it could be dangerous. (I am not a medical professional of any kind, just a layman, so take may advice for just that and do your own research and seek advice from your own medical care team.) I do know that taking antihistamines for priapism is recommended (sudafed and such). If you do, and then still need to go to an emergency room, TELL THE DOCTORS how much you took and when.

Don’t commit suicide! i’m 33 years old… my erectile dysfunction began in my mid-20s… within three weeks, I lost my vision, my job, my girlfriend, and went into congestive heart failure/kidney failure… for the following 3 1/2 years or so, I had absolutely no sex drive, no morning erections, and I was dealing with dialysis and loneliness… at my sisters wedding, I met a young, beautiful Vietnamese woman… she loved that I made her laugh and we danced that night… Things got serious between us very quickly… we began dating shortly after… I had to explain to her that because of my dialysis treatments and congestive heart failure, I was unable to have erections… We made it work for years! in October 2019, I got my implant… Until that point, she and I were only having oral sex and using sex toys to keep that part of our relationship strong… What I’m saying is that I went through an awful lot and Still managed to find love and get out of all of the bad situations. It seems really dark now but, let’s work through this! Let them do the psychological examinations and the physical ones so you can prove that this is not psychological… it may take a little bit of time but, trust me, this is so worth it… you do not need a penis for good sex… In fact, not being able to use my penis taught me so much about her body “more than anyone else has ever known“ and my own… we are in our early 30s now and have unimaginable sex… if you don’t feel like you are able to be in a relationship right now, focus on yourself for a while… find out some hobbies, stuff like that… all of us have had these challenges… You are going to make it through this! Please, be good to yourself, my friend… Love is alive… oh, fala Portuguese?

The priaprism probably damaged your penis due to lack of oxigen into the blood. try to use cialis / viagra and if finally you have to use a prosthesis that's not the end of the world.

I believe you can expect to be the same size after implant as before.
Please investigate less radical ways to treat your E.D. The results of your MRI should help clarify your choices.
I suggest using a vacuum device to keep flushing blood through your penile tissues (pump up gently to an erection inside the vacuum tube for 30 seconds, release the vacuum, lose the erection, repeat and repeat for 30 minutes, twice a day. That seems to be an effective protocol.) This also maintains tissue elasticity and size.
I did this for 14 months after my last real erection while awaiting approval for surgery and finding a surgeon to trust. I lost NO SIZE at all. My prosthetic-provided erection is fully the same length as my pre-E.D. erections.

VED use for therapy (not for coitus) was not thought of as effective by many top surgeons only a couple of years ago. Now, it is widely recognized as beneficial before implant and is even being considered useful after implant. But some medical doctors are slow to update their opinions.

I searched and found a chain of references leading to this protocol used to straighten Peyronie's disease sufferers' curvature. It recommends 10 minutes twice a day with maintaining erection for 3 minutes, release and repeat.
