First of all, when people complain their dick is flat, not round as before, what does that really mean..?
When is "before"?
Day before implant on Viagra, or day before ED?
A normal erection, completely without any other aid than a naked woman, or helped by Viagra, normally fills out both the cavernosum and the spongiosum. Hence your dick is round. And your glans is nice and full.
If you just inflate your implanted dick, and is not even turned on - the only thing that will fill up is the cylinders inside the cavernosum. Hence you are oval, because you miss out on the rounding effect of the spongiosum.
What if you are aroused? Well, then it depends on the ability for your spongiosum to fill up and stay filled.
If that ability was lost together with your cavernosum erectional ability, then you are doomed to have a flat erection.
That is not really the fault of the implant, the implant was never supposed to fill up the spongiosum.
That is the fault of your ED.
Without the implant you wouldn't have shit.
Now at least you have a hard dick from the cavernosum cylinders.
The flatness due to a non-working spongiosum is just something you have to accept. You lost it due to ED, not due to the implant.
Then of course, some get lucky and maintain their spongiosum capacity.
I did. When I inflated my dick without any blood flow, it looks not nearly as good as it does when I am horny.
Flat, small glans.
But when aroused, it gets round and my head grows and hardens.
Huge difference.
