Cycling 3-4x per day to maximize length / tips into glans ?

Postby ViaSwiss » Tue Jul 06, 2021 4:55 pm

Is there any downsized about cycling 3 or even 4x a day ? Cycling is getting less painful for me a few days in already.

Flaccid, my tips are not into my glans.

Pumped, the tips dont go far enough into my glans.

I want to maximize the tips entering further into the glans + adding length as soon as I can so I don't have floppy glans.

I may be wrong, but I don't think cycling will make the cylinders advance into the glans.

This is something that the surgeon should have addressed intraoperatively.

I don't want to worry you, so take my words with a grain of salt. I'm no doctor. If I were you, I'd consult another surgeon just to have a second professional opinion.

There might be a chance that your cylinders are still too firm to allow the slight stretch that you need. You're still early in the process. It is my guess that you need more time for the material to soften up a bit. Be patient & keep the forum up to date.
If six months or so passes by. I think you could get away with just a RTE added to the base.

I hate to say this, but I agree with Tortão. I know my cylinders were all the way in the glans from day one.
Did the surgeon use traction sutures and did you have "snake eyes"?

For my own sanity right now, I am refusing to believe to that my tips will not move any further at all into the glans. I do have the snake eyes and I know my urologist is aggressive in sizing.

But on the other hand I guess GT 1956 has a good point that is a bit relieving. Adding RTEs should be an easier "revision" if needed.

I really hope this turns out right.
It would be good to hear others who had the tips move into the glans.
I can only rely on my own experience which may not be typical
Good luck though.

I don't have a link to the story. But I'm pretty sure that I remember reading about a member that did have a RTE added. Vaguely remember one member that was going to have a RTE removed but I don't think he needed it done.
Just my non medical opinion here. This is another reason to choose your surgeon wisely. A skilled surgeon that knows where & when he/she can push the envelope is golden to have. I think Dr. Eid proves this frequently. I'm sure he isn't the only one that can make those decisions. Now finding one that can do that is the trick.
Don't let the little things bother you at this point. Best of luck to you.

My uro is a center of excellence for Coloplast, one of the few in the midwest. He does 300+ per year and his philosophy is to aggressively size. Even stated that was his plan right before the operation. Below was his response when I asked him about the floppy glans...I have to think I am overthinking this and all will be fine. Its just really concerning that most others don't have this issue early on..

"your anatomy was asymmetric distally. You actually had shortening of your left corpora compared to your right and your right was a little inferior and lateral compared to your left.

I checked several times and gently probed both measurements more than once to confirm. After some consideration I actually placed some additional rear tip on the left to allow that more ability to stretch and match the right with time.

You are 100% ok to start cycling. Regular stretching is going to be essential for your long term success. Also, what you see now is going to be different compared to what you see after some rehab and with sexual stimulation. Even if you took a viagra before intercourse to stimulate glans blood flow you’d see a difference.

Looking forward to chatting soon"

I believe Dr. Tatem is one of the top implant surgeons. He wouldn't tell you that the cylinders will go properly into the glans with regular cycling.
It is better to do cycle 30 mins in the morning and 30 in evening, which is the regular process, unless your doctor states otherwise. Don't get worried. It is too early to see the results.

I found the following 2 resources that may explain what is going on with me...

https://pubmed.ncbi.nlm.nih.gov/27872024/

https://www.youtube.com/watch?v=4kJeKhXWPCU

Results: In many cases of FGS, poor intraoperative prosthetic cylinder positioning and sizing can lead to insufficient compression of the deep dorsal and circumflex veins between the Buck fascia and the corpora cavernosa, even when cylinders are maximally inflated. When the adjacent tissue does not adequately restrict blood flow through these vessels, then glanular tumescence becomes increasingly difficult to achieve, particularly in patients with severe erectile dysfunction who have poor glanular blood flow at baseline. FGS also can be a result of poor underlying glanular structural support. Thus, droop is possible even when cylinders have an appropriate size and position. Treatment options range from medical management to surgical correction. Distal penoplasty and glanulopexy have been described as effective methods of correcting the FGS deformity and avoiding the more invasive option of prosthetic cylinder replacement. These options are especially valuable in patients who undergo adequate cylinder sizing intraoperatively.
