For future reference can people list the insurance companies that except IPP surgery as not an cosmetic enhancement and will cover with no hassle?

Medicare/TriCare for me. I told my uro (by email) about the meds (Viagra, Cialis, Trimix), acoustic wave therapy (including my own device), kegels, pumps (VED, Bathmate, air pump), HRT--I think my first words in person to him were, "I want an implant!" He filled in the rest for Medicare/TriCare.
Married 50+ years, 75, ED for 10 years.

Blue Cross Blue Shield/ Medicare Advantage. My doctor said they make him do a lot of tests before they agree to do it, his insurance office worked with them and I was approved. Only paid my copay for 23 hrs in hospital, less than $250. One bill I saw was $65k in 2015 dollars.

I'm not sure that an accurate list can be gathered. Many large employers will dictate to the insurance company things they'd like to include or exclude to help control prices.
I've seen guys get coverage & some get denied with Aetna. I have Aetna, but its a plan where Aetna bought out another insurance company. This company was bought & sold several times while I've had it. The original format was a HMO. I've talked to Aetna, they still call it an HMO. Either way, my surgeon says he has had them pay before.
My point is, there is too many plan options from any one company to make a blanket statement in my opinion.

34 yo, limited hand/finger dexterity. Tried vacuums, pills, TriMix, and Jelqs and nothing works. Implanted with AMS 700CX 21cm, Right 0.5 RTE by Dr. Clavell in Houston, TX on 5/12/21. Frenectomy 5/11/22 by Dr. Clavell.

I have Medicare Part A&B, Federal Blue Cross & Blue Shield and I didn't have to pay anything. I had same day surgery.

In regards to private insurance providers, it is not the company that decides if implants are covered, it is the individuals plans.
I did all the insurance set up for my company. In the application, your company gets an option to check a box that says sexual dysfunction is not covered. If your company checks that box, implant is not covered. I made sure to not check that box!

The thing is that insurance is state controlled. And employer's have options to have certain coverages. I buy my insurance and it is the same plan as offered on the Healthcare marketplace (Obamacare). It has coverage for physical ED caused by injury of disease. It is BCBSKS and is a EPO plan where BCBS select's the doctor in my state. And all the doctors are low volume and have very long activation time and expect size loss. And part of the preop counseling is that the results by the doctors are so poor that many men elect to have the device removed after the surgery.
So you may want to check with your employer to see if they have an exclusion. If so check if you can the plans available in your state. My BCBS plan is close to $1200 a month. That is 14K a year. There was another provider the had cost almost twice the BCBS plan and had out of state coverage. Given the cost for a policy to cover this you might consider a savings plan and looking for cash pay doctor. There are some good ones in the US for under 20K. Just depends on your location and if you have a very good doctor in your state that insurance will pay for if you buy your own insurance.
