I'm feeling the need for info Guys .. I ain't bragging but back before Implants were popular , I asked a URO Dr .this question . After my injury , I said "Doc , I know this sounds stupid but why couldn't they put a pump of some kind in a man's scrotum and figure a way to put some kind of fluid up into the penis to make an "Erection " ? Ok I didn't have all the details ETC, I was 21 yrs old . He laughed , said " Sounds Good ! maybe one day someone will find a way for that to work . Well, THEY DID !! I'm still waiting 51 yrs. on my "ROYALTY CHECKS " ..So far nothing .. When did Implants first hit the market , does anyone know . I'm sure somebody does . . My real QUESTION is "Why do they need to remove the Corpus and permanently disable the Penis . Since Blood is naturally pulled into the penis for a normal erection . Why can't they design a pump to pull the bodies OWN BLOOD into the penis to create a natural erection . Doesn't the blood flow thru a main vein into the penis with a natural valve to keep it there until ejaculation . They have a "man made valve to keep the penile fluid in the implants . why can't it use your own blood with a manmade valve to keep it in the corpus ? Ok , I need a SHRINK " .. But every invention starts with a dumbass idea . Who ever thought a man could fly , or that NASA could land a space probe on an asteroid moving thousands of MPH thru space millions of miles away . ? Is it because the "IMPLANT PROCEDURE " now used is so
perfected {AS WELL AS YOU CAN EXPECT } that Drs, are satisfied with things as is . Does anyone know of any new ideas of improvements being studied , or is THIS THE BEST WE CAN EVER EXPECT ?? IF YOU DON'T ASK, YOU NEVER WILL KNOW . I FIGURE THIS WEBSITE IS THE PLACE TO ASK .

All things seem to get smaller over time, look what happened with the VCR. I don't see why it's a dumb idea, you never know where the next great idea is coming from!

One of the great benefits of working with students is that they provide a fresh outlook and a fresh approach which has the potential to be novel and ground breaking. So students with open minds must be listened to.

Abstract: Erectile dysfunction (ED) has long been described by physicians and patients, with treatments for ED proposed starting in the 8th century BC. In the last 50 years, however, there have been many advances in medical and surgical management of ED, notably the introduction of the inflatable penile prosthesis (IPP) in 1973 and phosphodiesterase type 5 inhibitors (PDE5Is) in 1998. Here we review the evolution of the IPP from 1973 through the current day. The 3-piece device was first described in 1973 by Dr. F. Brantley Scott, who helped found American Medical Systems (AMS) to market and sell the device. In 1983, Mentor (now Coloplast) started marketing a competing device. AMS and Mentor have made multiple modifications to the device over the years, which have increased rigidity, durability and patient satisfaction, and have decreased surgical variability, post-operative infection and spontaneous inflation. Today, the IPP is a safe and effective option for many men who have failed medical therapies, with high satisfaction from both patients and partners.

With regards to the future, BIG SHOUT OUT TO DR HAKKY who has done some interesting work as described below:

".....In 2013, Hakky et al. created 3D models of human cadaveric corpora cavernosa and compared them
to two different models of Coloplast Titan IPPs to assess how well the device mimicked the natural penile anatomy. They found subtle differences in shape, especially at the distal end of the device. This is an area of ongoing research and remains a limitation of current IPPs (44,45)......."

with regards to your question on ligating veins, this has been tried but apparently does not work great.

The above have been vascular surgery attempts to fix ED (luke warm results, even from the reports here on franktalk)

The paper mentioned above does discuss other vascular surgery attempts (which, interestingly, was the first approach to fix ED) and those procedures just did not work out:

".....Physicians, realizing that ED could result from abnormal blood flow, attempted dorsal penile vein ligation and in 1902, Wooten attempted the first vascular surgery for ED, though this was unsuccessful (9). The first successful surgical intervention for ED was performed in 1935 by O. S. Lowsley. In what is believed to be the first penile plication procedure, he applied observations from his work with dogs and plicated the ischiocavernosus muscle and shortened the bulbospongiosus muscles of human male penises. He noted success and patient satisfaction over 10 years but cautioned that this procedure was for a specific patient population, namely men with a traumatic etiology of ED ..

Tangerine. Good post. Lots of people don't realize for how long the medical community has worked on ED. Including on implants. Its taken lots of work to get where we're at now.

Yes, in the coming years who knows what science will create? I do know some company is working on a remote prothesis. It will be implanted similar to what is currently done, but the pump and reservoir will be one piece implanted in the abdomen. The pump will be remote controlled via a key fob or a phone app. No more third testicle!
