Hi everyone,
I'm 25 years old, and I've dealt with pretty bad ED for my entire life. I was diagnosed with a severe venous leak in 2020, and I've been able to confirm that diagnosis with other urologists after that.
In a nutshell, my situation is that I can have penetrative sex without any pills etc., but only standing up, and even then, my erections leave a lot to be desired and start to go away as soon as I stop stimulation. If I lie down, the erection immediately goes away. Pills make my erections somewhat stronger but not enough to fundamentally change the outcome for me, i.e. let me have less constrained sex. Injections give me stronger erections, but they also go away quickly, and are very unappealing to me since I don't want to feel like my sex life depends on these shots. Moreover, based on my experience with them in doppler tests, it doesn't seem like they would actually help me have normal sex (i.e. lying down, etc.).
I have tried a barrage of different supplements over the years. They helped a bit but had no major impact on the situation. I also tried going on an Ayahuasca retreat to dig into my own mind and see if any emotional or physical trauma from my childhood had ended up manifesting as this venous leak (psychosomatically or otherwise). While helpful, I didn't find any connections there that fully explained or resolved the issue.
At this point, what's left to be tried (that I'm aware of) is shockwave therapy and pelvic floor therapy. I have had trouble finding these services where I live, but I haven't been very enthusiastic about the search because I'm skeptical about whether they would work or not.
I'm at a crossroads here. Based on the context above, if you were in my shoes, would you keep trying non-surgical treatment options or simply bite the bullet and get an implant? I'm of course concerned about the number of revisions necessary at my age. I worry about making this decision prematurely. I'm also hesitant because I can still have erections (albeit very mediocre ones). But I am not satisfied at all with the kind of sex I am able to have right now, and I'm tired. I'm tired from having dealt with this problem for so long. It has been 8 years of my youth so far. I would like to simply enjoy my life.

I feel you.. I am 31 and have been dealing with ED since puberty. Pills gradually lost efficacy, and just decided to skip injections (for reasons similar to what you described), and directly go with an implant. Surgery scheduled in mid March.
31 yrs old. ED from since puberty due to venous leak. Pills worked for 7-8 years and then stopped. Skipped injections, got implanted on 3.11.2023 with Titan Genesis pump 20cm+1.5cm RTEs by Dr. Levine.

So, if I were in your shoes at 25 and had the roughly $20k it takes to get an implant done now, I'd get the type/method of implant that I wanted, as soon as I could get it done by the surgeon of my choice ASAP.
IMO, there's no reason to delay otherwise.

Age 73. Started taking 5 mg Cialis daily in 2000. Minor ED started in 2021. Major ED problem started in 2022. Coloplast Titan (20 cm w/1cm RTE) implanted infrapublicly on 01/24/2023 by Dr. Edward Karpman (El Camino Urology Medical Group, Mt. View, CA).

Active, athletic 63 years old. Sexually, still 33 in my mind and spirit. Pills and injections all worked, until they didn’t. Diagnosed with veinous leakage in 2022. Coloplast Titan. 22 CM. No RTE. Peno-scrotal. Implanted 1/4/23. Dr. Clavell.

Thanks! I am from Chicago and will go with Dr. Levine. I asked him about VED and he said no need to do it since there is little evidence that it helps with size.

That wasn't what I meant to say [which I have sinced edited] which was that I had an excellent sex life until 70
. . . but I did 1st have sex at 13 because I was "advanced" for my age.

What’s up dude, your situation sounds a lot like mine. I got “erections” my whole life… they were just shitty af. You were only able to penetrate standing up, I was only really able to do so missionary. Lying down was out of the question for me as well.
Pills did nothing for me, and injections prompted a response, but still didn’t give me what I would consider a true proper throbbing consistently hard erection. 7 weeks out from surgery, I am still adjusting, but I don’t regret it man. The Titan at 50% inflated is harder than my old erections lol. And even masturbating or alone, it’s a trip to have an erection that you have confidence won’t completely disappear in a few moments if left idle.
I think you’re in your right to move forward to implant if it makes sense. Id consider the OTR if possible! The Coloplast classic I have quacks like a duck lol
28M, inadequate partial erections as far back as I can remember due to VL. Completely unresponsive to pills, disappointing response to Trimix. Stretch test 19cm. Implanted 1/6/23 24cm Coloplast Titan (Classic Model) by Dr Eid with .5cm RTE on left side
