I'm new here. Wish I found this site before my implant. Here is my story from my first 4 weeks. I went into a lot of detail. A little embarrassing to discuss this but if it helps, yay. I'll make updates throughout the first year. Here you go:
Penile implant experience
I am 58, 5’9”, and weigh about 260. So yes, overweight, but I work out so a combination of both :/
A little background. I tried the blue pill, worked a few times. Then the white one, good result with the first one but then again, stopped working after several doses. The doctor recommended the injection, Endex. First time, wow, 4 hours with 10mcg. Not so much the second time. The doc suggested 20mcg. Lasted 11 hours and was really painful. My penis felt like it would bust from the blood rushing to it. Ever inject a turkey? Like that. This hurt more than any surgery I have ever had (back and shoulder). Tried a few more times, but it stopped working and not only that, there was pain for days after the injection. Finally, the doc recommended the implant.
I’m writing this experience from a “normal” persons experience in the hopes it might help with some questions and preparations. I had the 3-part system put in. The procedure was accomplished Nov 21, 2023.
One week before the procedure:
I met with the doctors for any last questions. They told me to expect to spend the night, worse case, or go home afterwards. I packed for overnight. Told me to continue my blood pressure medicine but stop taking my blood thinner medicine 4 days prior. They also mentioned for me to buy some jock straps. I thought, ok, I’ll go to Walmart and get them the day before. Walmart didn’t have any my size. All youth! Wife ended up going to Academy and getting me one pair and one pair of compression underwear. Now those are awesome. So, don’t wait until the last minute to get those items! Yes, I recommend getting both, or at least Jock straps (around 3-4), and underwear that holds your stuff.
The night before I had a 4oz steak and broccoli. No alcohol. I stopped eating at 6pm even though they said to stop eating by midnight. I’ve had surgeries before and leaned to the conservative side.
Day 1: The day of surgery:
Last minute questions from the staff and doctors. Ended up with IVs in both hands. On the top. Antibiotics were started first. Don’t forget to go to the bathroom before because you are now stuck in bed. They gave me the good stuff and wheeled me in. Don’t remember getting 10 feet past where I started.
I woke up feeling I had to pee. I tried reaching down to hold it to pee but they wouldn’t let me. They said they would hold the plastic urinal for me to go. The pain was like holding it too long. Ever been driving and have to pee but the next exit is 10 miles away? That’s the feeling. Later I realized it was just a feeling since they had placed a catheter so my bladder was technically empty. I looked down to my penis and it was completely wrapped with gauze.
An hour later I told the nurse I needed to go. I gently peeled back the gauze to expose the tip. Peed about 400 ml. Also leaked onto the gauze. That was going to be a problem for me since they said not to remove it for 24 hours. Ugh. Peed again later and same result. Pee in the bottle and on my bandages.
They had also put me in a jock strap. Everything was numb except the feeling of having to pee. That part sucked. I also wore sweatpants. They will leave your penis partially inflated to help with the stretching of the penis.
I did well enough where they sent me home that day. Yay! I think my wife found all the bumps on the way home. LOL. No problems walking. Just took smaller steps. And walking upstairs to my bedroom wasn’t a problem.
Before leaving, we asked for some gauze. They gave us some 4-inch squares. And asked if they had any large bed wetting under pads. Get several if you can. One for your bed, and one for the chair you will be sitting on when you get home. Remember the wet gauze?
So, I’m home and have to pee. I sit, gingerly, down. Then have to gently move my penis under the seat to pee. I held it down and peed. Yay. Then, let up a little and more pee but some came shooting out between the seat and commode. Stop laughing. At least my sweatpants were there to catch some of it. Later I would grab some toilet paper, about 5 sheets long, folded it lengthwise 3 times and set it between the seat and commode. Victory. Also had to be gentle taking my penis back out. You’ll figure it out. Oh, gauze was nearly soaked by this time so I squeezed out as much as I could before getting dressed again.
Make sure you write down the times you take the medication and keep on schedule. Staying ahead of the pain is important. Body heals better when not in pain.
Took more meds and went to bed. Have someone help lift your second leg as you lie down. You can probably do it but it just helps with the pain in your stomach. Took my meds and went to sleep with a wet crotch.
Day 2:
I had to get up around 3 am to pee. Was still in some pain so I was glad my wife put out the pain meds for me to take.
I got out of bed and went to pee. I told my wife the gauze was pissing me off, literally. I took off the gauze, cleaned up gently, then put about 4 gauze rectangles (I unfolded the square) around the penis area. Then wrapped it with about 2 feet of toilet paper. I didn’t have any tape so I used 2 bandages to hold everything on. Later, my wife went to CVS and purchased gauze rolls and the tape to hold it in place.
Going to the bathroom was getting to be a pain when I realized, I have a walk-in shower! I stood at the end and peed inside. Ever tried peeing with a hard-on? It’s like that. It was awesome. Afterwards, I turned on the shower, drained everything, then sprayed it down with disinfectant. Tried to poop but nothing. Keep taking that Colace, it helps keep everything soft for when you do go. (in a couple of days).
By the way, when I first took off the gauze and saw my penis, well, ever see Rocky and how his face looked like at the end? This was worse. All bruised up. Like being a prize fight. Don’t worry. It will get better. I was told to have my penis lie upward against the belly but towards the end of the day, it was pushing against that band at the top of the jock strap. I changed into the compression underwear. Wow, way better. Especially at night. Jock during the day, compression at night. My ball sack was completely stretched out. It felt the size of a tennis ball. No pain, but big.
Day 3:
Still can’t poop! Was finally able to shower. Walking is still good. Just felt like getting kicked in the Jimmy! Waited a little too long for my morning meds. Thought I could handle it. Nope. Don’t be stubborn. You know who you are…every guy out there! Eating a little less than normal but still trying to stay hydrated. Penis looking a little better. That evening, there was no more drainage so I didn’t use any gauze. Went for small walks around the house.
Day 4:
Much better today. Penis still hurts a little so staying on Tylenol. That seems to be helping. I would also get little jolts of pain near the right side of the head. Realized it was probably from the plastic piece inside trying to “fit” in. Massaged it a little so the tissue inside could get used to the placement. I think. Finally pooped! In fact, ended up going about 4 times that day. That’s the only time I’ve used the toilet instead of the shower. If you don’t have a walk-in shower and want to pee in the tub, please be careful. Still waking up at night to pee but most of the pain is gone. And still taking Tylenol before going back to bed (my wife put extra in the bathroom). Ball sack is deflating and I can feel the pump location. Not trying to inflate or deflate yet but getting an idea where it’s located.
Day 5:
Woke up feeling really good. Still woke up at night and took my meds, only Tylenol, but moving around has gotten a lot easier. Penis is starting to look way better. They left it about half inflated so now looking like what I was used to see. Except with some minor bruising. Ball sack almost to normal size except the pump. Overall, feeling pretty good.
Day 6 – day 14:
For 2 days in a row, I was just using the support underwear. Bad call. It. didn’t really give the support I required. Went back to only using jock strap. Much better by the second day. I would suggest only using jock straps for the first two weeks. Don’t man up. Use the jock straps. The sooner you heal…well, you know.
Lessons learned:
Buy 3-4 6-8 jock straps early (lesson learned)
Buy some support underwear
Buy rolled gauze and tape
Stay on top of your meds, especially Colace and pain. Don’t be a man about it. You paid for it, take it.
Toilet paper between the seat and commode for the first few days
Buy sanitizer spray for after you pee in the shower

Day 15: Meeting with the doctor
My wife suggested I take some Tylenol before going. Good call. Should have taken a little more. The doctor showed me how to inflate/deflate. At first, a little trouble standing up so I laid down on the med bed. A little easier. The first doc, primary, had a little trouble with finding all the parts due to I was still a little swollen. Then asked the other doc, training in progress, to feel and try. Ouch. A little rougher. 1st doc showed me again and way gentler. Then had me try. Pumping portion was tough. Had to squeeze the bulb through a tender sac. Was only able to inflate some, not all, to make sure it was working for me. The relief valve was easier. After the meeting, I forgot to ask when I could start working out again. I’ll let you know. Walking back to my truck and the rest of the day was rough. My sac felt as though it were a punching bag but not too bad. Took more Tylenol before bed.
Day 16:
Woke up feeling like I got hit in the Jimmy. Sac was tender. After work, I took Tylenol, then attempted to cycle the inflation and deflation. OUCH. Afterwards, felt like a 5-year-old jumped on my balls! It felt like it did the first day after surgery. Felt the need to pee even though I just went. I didn’t deflate as much as I should have so…peeing with a hard-on, again. I deflated a little more. Much better. Took some oxycodone. 50 minutes later started to feel way better. Recommendation, save some of the good stuff from the surgery and take it about 30-45 minutes before attempting to cycle the first time at home. I’m sure it will be better tomorrow. I know it’s a pain right now but knowing it will be worth it later, suck it up. And take the meds!
Day 17:
Hope I didn’t freak you out from yesterday. It freaked me out a little. I woke up about 20 minutes before my alarm went off. I thought, do I need to take some meds? Well, no. I reached down. My sac was not swollen. I thought, what the hell, let’s give it a try. I inflated, ok. Then deflated. Yes, scrotum was sore but not painful like the two days previous. Yay! Got up, showered, went to work with some Tylenol.
Day 18 - 20:
Much better. Sore, but no real pain. Just keep inflating and deflating once per day. It gets a little sore with full inflation, but massaging helped. (yes, only massaging for a few seconds). Then deflation was much easier. I can even do both while standing up. Still only wearing jock strap. Tried the underwear overnight on day 19. Ugh. Not quite yet.
Day 21-28 (week 3-4):
Even better. Cycling twice a day. In the morning after going to the bathroom and letting it stay full until the end of the shower. Then once in the evening. I tried normal underwear for a whole day and overnight. Nope. Keep using the jock straps. I was really sore the next day. Ordered more jock straps since I only had 4 and wife didn’t like washing every 3 days.
Week 4:
Cycling and deflation is getting easier. Just remember to give it a quick squeeze after deflation to pop the valve back in the proper place. Don’t worry, I don’t think it will inflate on its own. I have noticed that even fully deflated, I’m about 1/2-3/4 erect but still easy to hold to point down to pee. Guess the plastic inside can only get so small and vice versa so big. I think in total, I may have lost 1/4 inch but I think worth it since I don’t have to worry about it not staying hard the whole time. Guess I’ll have to learn some new techniques to make up for the small difference. LOL.
I have also noticed I need to pee a little more often. At least right now. You know how you can “hold it” a little longer to pee? Well, when I do, my penis seems to hurt. After I pee, no more pain. Maybe because it’s only been a few weeks or the muscles aren’t sure how to hold it a little longer. Just go pee. I think it has been getting a little better to hold it as the days go by. And looks like all the stitches have healed.
Here I come to the big question. Can I still ejaculate? Being a guy, you know about waiting. Before I mention the experience, I would like to make an observation. I’m not a doctor and not sure how the mechanics work precisely. When you cum, your balls squeeze it out quickly, then the rest of the muscles in the penis push harder to get it out as fast and far as possible. With the plastic, those muscles have been mostly (not all), replaced. So I did what every guy does, occasionally. I put on a movie and got tissues. When I came, it was the same feeling as before the surgery. It felt like it shot from the balls, then slowly made it way out. Felt weird. Guess the muscles couldn’t push it out as fast as before. Tried it again a few days later and same weirdness. Not bad, just weird. Looking forward to seeing how it feels when the wife and I can start making love again. Two more weeks!
I feel totally dumb mentioning all this but can’t find anything online discussing any specifics and expectations. I just hope my experience helps in your decision, expectations, and preparations. Good luck!
It's been over 2 months. It's great not having to worry about getting it up. I'm wearing out before it does! LOL! Wife is really enjoying it. Wish I would have done this 10 years ago. If the pills don't work, skip the injection if you can. I know it can be expensive for the surgery but may be worth it in the long run.

Good read Texas Dad. I’m 58 and had mine 20 months, very similar experiences. I pumped at 13 days and had sex at 3 weeks. Was sore afterwards but well worth it. Hang in there and keep cycling. Might add some perito exercises to help the break in process. I was about an inch short immediately after implant but by 6 months was only a 1/4 short. By 10-12 months it was all back to full length so don’t sweat the small stuff. Look forward to hearing the progress! Welcome to the bionic brotherhood!
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast 20cm and 1cm RTE. Going strong and loving it!

Texas Dad, thank you for the great details. I have had several interviews with my surgeon, with over 50 questions. Still nervous as hell. However, reading your post details, I feel much better. Still nervous, but not shaking as much. I like the part about your wife hitting every bump when driving, I know exactly what you mean. My wife too, has to hit every bump. It's her nature. She can't miss any bumps in the road. Again thanks for the read.

Good info for others considering an implant.
All of us have gone through some issues, pain, and WTF have I done moments. It's all a part of the recovery from the surgery, but so worth it once all is healed.
