Query on Sensations / Hardness over time (post-op). Possible injury?

Hi Guys,
I wanted to ask what may or may not be a question relating to my implant (I'm truly not sure) to gain your input.
I have a titan implant and following the recovery period (both physically and mentally) have been seeing a lot of success especially when paired with 5mg of Cialis. I state success in basing it upon my penis still engorging when aroused (especially after several days of abstaining from masturbation), the head becoming full and hard, and the titan lending structural support as a scaffold; no issues with having sex and often the Cialis was unnecessary following the aforementioned periods of abstention. This had been the case for a few years now.
Fast forward to recent months and a change has occurred which I can't figure out.
All of a sudden it my erections / engorgement have become lesser. Including sensation. My titan still functions perfectly well but I'd swear that I can notice a difference in the blood flow to my penis, and even feel the difference as though the shaft has suddenly lost natural girth due to lack of pressure. Erections are suddenly not what they were.
When taking Cialis I no longer feel the constant 'pressure' in my penis throughout the drug's duration of effectiveness either. Morning wood is sporadic and with lesser feeling / strength, and there's no guarantee of me being able to perform.
All of these changes appear to have happened within a relatively short space of time and I'm wracking my brain trying to figure out if I injured myself somehow though there's nothing I can think of. Lifestyle changes have included some long runs as I picked up my exercise routine though not to an extreme degree.
Now, I should say that this change has overlapped with what I can only describe as a period of poor lifestyle in terms of diet and lack of sleep, stress etc. I was travelling extensively for work with little sleep for 2-3 months and my diet was shocking, steaks, booze and so on. For the first time in my life I actually experienced high cholesterol (I regularly get check ups on my health) which is of no surprise.
Now, already my lifestyle has reverted to a healthy one, and my cholesterol is returned to a perfectly healthy level as I cook, sleep, exercise, and eat well once more. I'm also 'resting' my penis and trying to be kind to it, but thus far no positive signs have appeared of its functionality reverting to what it was.
Any ideas? Injury? Further rest? Cholesterol and the extremely unhealthy lifestyle are responsible? Do I just need to wait longer? Something else?
Am really keen to hear your thoughts as some may have already experienced similar, and at same time I'm now reaching out to the consultant who performed surgery on me to see what he says.
