Hello all. I've had my implant over 6 years now. Wow. Still love having it and glad I had the surgery.

When I'm very aroused, I still get a small amount of chubbiness in my shaft, though not much. Not a problem, just an observation. I can get all the erection I want with the pump.

I also get some glans engorgement. I have heard that the glans is fed from tissues on the underside of the penis, not affected by the implant surgery. But I don't get a lot. I was discussing this with someone in chat a couple months ago and he told me his uro had prescribed a trimix(?) cream to apply to the head and it worked to make the head of his dick significantly larger. I asked my urologist about it and he had never heard of such a thing. I also looked online, but only see those overbearing websites offering penile enlargement.

Do any of you know of a cream that works on the glans to increase engorgement? I'm looking for something proven and given by prescription not a hype machine enlargement product. If I had the name, I could probably get my uro to write the prescription.

It's possible to DIY it by mixing crushed misoprostol pills with water and applying that as a paste to the glans. I don't think it would be available in the US as a prescription. In Europe you'd be able to get prescription Vitaros which is basically what I described in prescription form.

I had posted some time ago about Trigel. I used it and it worked but I stopped because there is 1 ingredient in there that causes pain. Even using the smallest amount, it helped the floppy head but still too painful. 2 years in and my floppy head resolved itself. Its very expensive and if you try it, start with the tiniest dose then adjust if necessary. You put the Trigel inside your peep hole. Only compound pharmacies can make it for you with a dr's script.

Take a look at this website for a compounding pharmacy in San Diego. They ship to most states in the US. The cost is about $150 for 3 ml of their "trimix" urethral gel. It's not trimix in the usual sense because it just has alprostadil and phentolamine and lacks papaverine; the third ingredient is lidocaine. This is fine because the papaverine in Trimix probably doesn't add much, and the lidocaine prevents the gel from causing painful burning in the urethra. The alprostadil component can still cause the same ache in the penis that injections do, but for most people, it doesn't happen, or it's mild enough not to be a problem. From what I've read and been told by compounding pharmacists, it helps with glans engorgement about 50% of the time. Some men find it also helps improve glans sensitivity if that was a problem after the implant. The effective dose can be as little as 0.1 ml, which would be $5 per dose, but it could require more. The expiration date on the syringe is 3 months after it's shipped. They make "super strength" versions if the regular strength version doesn't work well enough at the maximal dose of 0.5 ml. As a gay man and I've been using Trimix gel for several years to masturbate with. I get it from a local compounding pharmacy in Virginia.The last shipment was 5-1ml syringes for $125.00 and I inject about .15ml every time I jerk off. Their's a little bit of glands pain but not as bad as Intracavernosal injection with Edex, which is pure alprostadil (prostaglandin E1). Ask your doctor about it and perhaps give it a try, you may like it.

The degree of tachycardia you experience is a pretty unusual reaction to trimix injections or urethral gel. It could be from the phentolamine. Phentolamine is an alpha-adrenergic receptor blocker. When administered into the corpora cavernosa (either by injection or by absorption from the urethra,) the effect is smooth muscle relaxation and increased blood flow, aiding erection. When phentolamine is given intravenously in a sufficient dose, it causes a drop in blood pressure, which the body responds to with an increased heart rate. However, the amount of phentolamine absorbed systemically from an intracavernous injection would not usually be enough to cause tachycardia. To me, this suggests that you may have some underlying tendency to hypotension, or there could be some interaction of phentolamine with another medication you are taking. I'd recommend you review this with a primary care doctor/internist.
