Like the title says. I’m seriously considering an implant.
I started on pills at about 45. They worked great for about 5 years. I then switched to trimix and it was awesome the first 6-12 months. But now I am starting to get some hour glassing with the trimix and I am using it only once every week or two. I have always been very sexually active and have a great wife. She has been so supportive and understanding but I feel like a failure not being able to perform like I used to. I am in my early 50s and I was planning on dying old and in bed having sex. I am not overweight or unhealthy. I have been dialing in a better diet and have started to exercise more. I have a fast paced stressful job but the financial benefits are very good so I don’t want to change that. I am terrified that this will go wrong or I will make the wrong choices in doctors or devices. I have a million questions and no one to ask. I am still able to have erections with pills and shots but I know they are becoming less effective. I want to get ahead of this before I go years not being able to enjoy sex with the wife. I’m outside of Atlanta and appreciate any input. I am 7+ inches long and 6” girth. I want to act before that starts to decline. The hour glassing already has me worried. I’m a grower of that matters. You and I are in the same boat, and the same area. I'd recommend giving Dr. Hakky a call at Atlanta Cosmetic Urology, he's a very high volume implant surgeon, and who I plan to have doing my surgery. He's local to you which is a big plus, and he's easy to talk to.

Been doing injections for a decade, they work but I'm kind of over it and want things to be more natural and spontaneous. I'm about 90% sure I'll schedule a surgery in the next 6-12 months.

Very little of what can go wrong is catastrophic. Basically you can get an infection or there can be some sort of device failure. But in those cases, generally you can still get an implant, you just have further procedures and healing time. Infection risk is cut down dramatically by the experience of your surgeon and quality of the surgical center.

Doctor choice is relatively simple. You want someone competent that you are comfortable with. A lot of guys will recommend the super-high volume surgeons that perform hundreds of implants a year. If you feel comfortable with one of them, great. But studies have shown that surgeons that perform a few implants a month have similar quality outcomes to the ultra high volume docs, so they should be an option too, particularly if an ultra high volume guy is not possible do to distance or insurance issues. On the other hand, doctors that rarely perform implants - a few a year - should be avoided if at all possible.

Device - talk to your doc about this, rather than being swayed by what you read here. All of them do the same thing: give you a hard cock. The choice of one or the other is largely driven by sizing and other medical concerns. That said, I think there is more room for personal choice between inflatable and malleable devices. The vast majority of dudes in the US get inflatables, but the minority that gets malleables are very vocal on this board about why they think they are better. Again, you are probably best served by making a decision consulting with your doc.

This site is a great place to get some insights, find camaraderie, and understand that you are not alone… a lot of us have walked this road before, and the vast majority of us are very glad that we did. Satisfaction rates with implants are very high compared to injections or pills. But your doc will be the best place to get definitive advice on what works and what doesn’t.

If you want somethin that just works and don’t give you no trouble, get yourself a malleable. Ain’t fancy, but it’ll git the job done every time.

But if your feelin lucky and wanna take a wild rollercoaster ride full of ups, downs, and expensive pit stops, go on and get an inflatable. It's not fun crying when it leaves you stranded, unless sure you have good insurance, health and bank balance for inflatables and their subsequent revisions.

Do the malleable implants make you look hard all the time. I am leaning that way just to have the dependability and simplicity of the malleable. I am naturally 7” length and 6” girth. Will a malleable allow me to keep most of that ? Are they rigid enough for anal sex ? I’m pretty sure they are a once and done thing. That sounds very appealing since I’m only 52. Can an inflatable be used if I change my mind later ? Thanks for the feedback.

Yes, if you let it hang freely. It’s an adjustment curve to figure out the right clothing and positioning angle that’s most comfortable and concealable.

A seasoned surgeon can answer this by evaluating your anatomy against the available malleable rod diameters, which cap at 13mm. If they determine that your corpora are much larger, a good surgeon will warn you about potential girth loss if residual engorgement can’t compensate, or they might even strongly recommend an inflatable.

Length is not a problem.

With lube, yes.

I would again reiterate that you should talk to your doc about this. The vast majority of guys get IPPs, nonetheless there are a small but vocal number of malleable enthusiasts here that IMO tend to overstate the negatives with IPPs and understate the negatives with malleables. It may be that a malleable is right for you, but I personally wouldn’t lean one way or the other based on limited feedback from all us anonymous dudes that mostly don’t know much beyond our own experience. A good doctor should be the primary source of your advice on this, not us yahoos.

I’m a big advocate for implants. But which kind is right for you is way above my pay grade and everyone else here.
