What happened to Surgeo.com - any alternatives?

There used to be a website called Surgeo that allowed you to choose to pay for implant operations oeprations. It let you see out of pocket costs for different surgeons and provided a certain amount of liability insurance as well in case something went wrong. Looks like the site no longer exists..
