Background: My ED began in 2015 following a neurotoxic fish poisoning that wreaked havoc on my entire body. I spent 21 days in the hospital with a potentially fatal condition and saw a neurologist for years afterward—truly a major life event. I took oral medication for approximately 9 years, followed by 1 year of Trimix injections, before deciding to get the Coloplast Titan implant with Dr. Charles Osterberg in Austin, TX.
From everything I've heard and read, Dr. Osterberg and Dr. Bryan Kansas are the top specialists in Austin. Both work for Urology Austin, and the South location is conveniently situated directly across from St. David's Hospital, where they perform the surgical procedures.
Day of Surgery: September 8, 2025
Pre-Surgery (7:30 AM Check-in for 9:30 AM Surgery):
Preparation:
- No food or drink after 6:30 AM (only Gatorade or water allowed before then)
- Check-in Process: Extremely smooth—they had texted me a QR code a couple days prior, allowing for express check-in
Nursing Care: My nurse, Charmaine, was outstanding and handled all prep efficiently
Medications: Once the IV was placed, I received two antibiotics intravenously, plus Tylenol, gabapentin, and Bactrim orally
Surgery Consultation:
Right at 9:30 AM, Dr. Osterberg met with me to explain the procedure in detail, set expectations, and confirm the surgery would take approximately 90 minutes with same-day discharge. The anesthesiologist followed up with a thorough medical history review and assured me I'd be in excellent hands. Overall, the pre-surgery experience with the entire team—nurse, doctor, and anesthesiologist—was exceptional.
Surgery and Recovery:
- After being wheeled into the operating room, I was under anesthesia quickly
- Upon regaining alertness, nurses confirmed the surgery went smoothly with no complications
Post-surgery setup: I had a catheter, extensive gauze supporting my penis and scrotum, all secured in a medical jockstrap
Pain management: I received oxycodone initially, but the discomfort was primarily from catheter positioning rather than surgical pain. Once they repositioned the catheter, pain was minimal
Hospital stay: Rested for approximately 5 hours before discharge
Current status: Aside from the catheter issue, I've experienced surprisingly little pain and have been managing well with just Advil (beyond the initial oxycodone dose)
Medications to pick up: Toradol and Bactrim prescriptions
Follow-up: Post-op appointment with Dr. Osterberg scheduled for Thursday (3 days post-surgery)
I understand that recovery pain may intensify based on what I've read here, but so far, the experience has been more manageable than expected.

Congrats and best of luck over the next couple days.
Im exactly 2 weeks post op today and the first 3-5 are rough but im here to say its 100% worth it already.
Give yourself time to heal. You really will only feel like THIS for a very short amount of time. There may be challenges on the road to recovery but be proud and be patient. You've made a huge leap forward to reclaim your sexual AND mental health. In no time at all you'll feel relief from the dark cloud of ED.
Since my surgery ive been tackling life with new energy and motivation. Im now driven to get through my todo list. Im also motivated to be right in my relationship with my wife in a way I haven't been in years!

Which install did you have penoscrotal or infrapubic?
Did the doc tell you your internal measurements yet? If he didn’t, it will be on the surgery documentation.
Also, did he say what size implant he put in you?

Install was penoscrotal.
I'm not sure of measurements or implant size yet. I have an appointment on Thursday morning and will find out all that info.
Also, in response to your other post... I got the neurotoxic fish poisoning eating a swordfish steak in Turks and Caicos. Poisoned by the coral reef.

Happy to hear about your new lease on life from a mental and sexual standpoint. It will definitely be nice to be able to be spontaneous again and to not have to worry about all the mental gymnastics associated with ED. I definitely won't miss the Trimix needles and carrying that around, etc...
I guess I'm technically starting day 2 and still not much pain to be honest. I thought the pain would be much worse. Just using Advil and getting through. Hope it remains this way from a pain standpoint. I know I've seen some people say their pain levels picked up on day 3.

Thanks for sharing this RobotRomeo! I was diagnosed with venous leak recently and am currently in the process of deciding between the Titan or going with a malleable. I look forward to reading your journal as it will really be helpful with my own decision making too.
