Implant Girth Change

Hi guys - 35 year old guy from Ireland here. Veinous leak for 2+ years now after taking dutasteride for hair loss.

Ever since the leak my penis has felt different, especially at the base, it's been less substantial and more like rubber.

I had a vein ligation done in April 2025. It helped for about 4 weeks then the improvements faded.

I first tried 10mcg alprostadil in October 2024. It worked well maybe twice. I then moved up to 20mcg alprostadil, that has been hit and miss, giving a good result maybe 50% of the time. I'm now on 40mcg alprostadil and it's much the same, it's been hit and miss.

A few weeks ago I noticed my alprostadil injections were giving me great results. I injected about once a week for 3 weeks and the results were fantastic. But last night I took a shot and it didn't work at all. I'll take a couple more shots over the next 1 - 2 weeks and if they're not perfect then I think I will need to book the implant, I think if the next few shots fail me then it is probably fair enough to throw in the towel and get an IPP.

I'm obviously worried about losing size with an IPP.

Has anyone here ever lost girth with an IPP?

I haven't, I gained some.

42 ED for 9 years vl after a fall. Pre implant 8 1/4 bp x 6 1/8 ish
Clavell titan 26+1 rte…post op very excited: 8 5/8” x 6 1/2” mid and way over 7” base. (We’re up to 6.5” girth!!!!)

I portably lost about 0.1", but I guess I was on the slightly larger side at 5.5" girth pre-op. See full stats in signature.
36YO, ED for 15 years, unknown etiology. Based in Canada.
Implanted 10-Sep-2024 by Dr. Clavell in Houston, TX.
24cm Titan, OTR (one touch release) pump, no RTEs.
Pre-op size: 6.25L/5.5G
Post-op size: 6.3L/5.4G (so far)

I have the same length as before, maybe even a bit longer now.

Girth is much more than before, more than I ever had before with my natural penis which is a nice plus. I enjoy it.
42y/o, VL due to Finasteride use 17 years ago, PDE5i lost effect over the years, nothing worked anymore.

The answer is it depends.

Guys with average or smaller girth have a better chance of maintaining their girth. The guys with very wide, above average girth have a better chance of losing some (although they will still be plenty big).

It depends on the skill of your surgeon.
It depends on which model of implant you get.
Using a VED consistently for a few months seems to help surgeons place the largest implant possible for your anatomy.


So if you decide to go with an implant use a VED in preparation, go to the best high volume surgeon you can find, and talk with them about your concern.

It’s a pretty common concern. Nearly every guy wants to Maintain (or even regain lost) size.

I was implanted in March and had a large (6.5” midshaft, 7.5” base) girth. My surgeon warned me me I would lose some girth despite getting the largest possible model (Coloplast Titan). He said I would end up at 6.1”. So far I’m at 6.25” and may yet regain more.

The last pic in my implant journal (link below in my signature) is a side by side comparison of preop vs 6 months post op. The post op pic looks thicker although my shape is now a little more oval instead of perfectly round, still normal though. So it’s actually 0.25” thinner despite looking wider.

Regardless I’ll really happy to have absolutely dependable erections whenever I want, as long as I want, as hard as I want.

I see. Interesting. I notice your profile says you got veinous leak from finasteride?! I'm in a similar boat - veinous leak from dutasteride.

What was your penis like after the finasteride affected it? Did it change in texture or composition?

Mine seems to have mutated near the base. It sometimes feels rubbery and unhinged there. It's an extremely strange sensation.

I didn’t see any change in texture or composition. It just didn’t get hard anymore.
