Numbness risk - infrapublic or penoscrotal better?
I am going to have surgery this year—my biggest fear is permanent numbness, which is sometimes reported here. Does it make a difference in terms of risk whether you have infrapublic or pensoscrotal surgery?
49, healthy, fit. Mild-moderate ED for 10 years due to nerve damage. On 5mg Cialis daily. Looking for a miracle cure.

Using a high-volume, dedicated implanter reduces risks to 1%. Zero risk is impossible with any invasive surgery.

My understanding is that penoscrotal is less risky for possible nerve damage. I had penoscrotal and I have slight numbness at the incision and just above it slightly affecting the shaft but not noticeable during sex. Only to the touch. Doc said give it about 6 months for feeling to return fully.

I think it's definitely more common than officially reported. I have numbness on underside from penoscrotal, but I still have enjoyable sex. I think dorsal and glans numbness is more problematic.. and is more likely from infrapubic.

The general consensus seems to be that infrapubic had a slightly higher risk of nerve damage than penoscrotal.
But as others have said the biggest factor in avoiding surgical complications is to use a high volume surgeon who has done a TON of these. You want them and everyone around them doing this routinely.

I have to agree with you. I'm 3 months post surgery and have the same numbness. The lack of sensitivity can make me last for up to 2 hours without ejaculation. Wife's not complaining but is frustrated that I don't. I've been explaining it will return in time.
67 year old, happily married to my high school sweetheart. ED for 26 years with maybe a 15% success rate for pills. Couldn't see myself doing injections so took the plunge on September 5, 2025. I have an AMS700.

The thing to remember is nerves take a long time to heal or regenerate. No way to make them heal faster.

i initially had infrapubic and switched for a penoscrotal with revision surgery.
I had some numbness around the incision for about 2 months and some reduced sensation to the glans for 2-3 months after the revision surgery due to the tunical expansion, but all returned to normal at around 3-4 months. Agree - nerves take a while to wake up after surgery, but generally i think high volume surgeon will be careful with the nerves with ever approach.

This. I had penoscrotal, and after 9 months still have some residual numbness on the lower shaft underneath, near my incision.
It has very, very slowly improved over that time and I believe is still healing.
It's not a major issue for me as I still have full sensation where it counts.
71, married, Sydney Oz. PC/nerve sparing RRP Mar 22 then profound ED. Tried pills, Trimix inj, focal shockwave, VED.
