How do you tell your employer that you need time off for penile Implant surgery when you don't want them to know you are having it?.

How about "I'm having corrective surgery." "Corrective" suggests that nothing life-threatening is going on; "surgery" implies sharp instruments and sutures which mandate more than 2 days of recuperation. The rest is nobody's business, especially an employer.
Just my thoughts.

I used the hernia surgery excuse. I thought it best because when you return or if you return to work early as in 2 weeks (which I did) then it will make sense that you are sensitive in the groin area.

Just use the word "male surgery" and that covers a lot of territory.
If your boos is a friend you can trust, then you could share the real reason. That is your call.

If you know the boss well enough to tell him. Tell him straight up and watch him grab himself and tell that that is too much detail. Their expressionis priceless.

Good question.
My employer have sick leave policy but if it is more than 2 days one has to provide a doctor's note with the explicit description of the diagnosis.
Besides the HR office is located in another city so the papers have to be scanned and sent out electronically. That just makes me sick thinking that my most private information is going to travel through the internet and some HR personnel is going to judge whether or not I deserve paid days off...
