Really Depressed About Implant After Surgical Consult

Hi everyone.

So I’m about to have a prostatectomy for prostate cancer.

The good news is the cancer is very localized, and removing the prostate will likely eradicate the cancer.

But here is the crushing news (for me, anyway).

I had a slightly about average penis size, 6” length and 4.75” girth. When flaccid though, it was a little over 2”.

Over the course of four years (before my cancer diagnosis,) I increased my penis length to 7.5” and girth to 5.7-5.8”, using a penis pump. Flaccid length increased to 3”.

I was SO happy; I succeeded in doing something I was always told was impossible, to actually grow my penis.

But at my pre-surgical consult for a penile implant, the doctor said that the implant will AT MOST, only give you the size of your flaccid stretched length. He stretched my penis out, and it only stretched to 5”.

I told him my story, and that I’m a MASSIVE grower. I said that after using a pump, my penis is almost 8” in flaccid length, but over the course of 24 hours (until my next daily pumping session,) my flaccid penis shrinks down to 3” again.

He said that he CANNOT put an implant in THAT IS ANY LARGER THAN MY STRETCHED FLACCID PENIS.

I am crushed! I know that there are MANY men who have a 5” long penis, or shorter.

But I’m just heartbroken about this possibility.

I talked to a SECOND surgeon. He told me the same thing.

I’ve watched videos from Dr. Perrito, Dr. Eid, and many famous/high volume surgeons, and they ALL say the same thing— they will only implant a prosthesis which is the size of your stretched flaccid.

I feel like I was absolutely cheated in life by being a “grower,” not a shower.

I know I SHOULD be thankful that I will be cancer free, and have “only” a 5” penis. But it’s my body, and I feel like absolute shit about this.

Can anyone relate?

Yes Niculus, many of us have. I was a "grower" ("shower" post implant) and can relate to your story as mine was similar.
I also had very localized PC, then prostatectomy, and been 100% cancer free for over 4 1/2 years.
Best wishes for your surgery, it is a big deal, and recovery/side effects can be challenging for some of us.

Unfortunately your implant expectations are not realistic if judged by VED size. I also used a pump after the prostate surgery as I had started to lose size to ED/atrophy. I could also get a massive increase in length and girth from pumping, with basically the same dimensions as your experience. But VED size is only artificial and temporary. Yes it can seem like you have permanently gained if pumping aggressively every day. But if you stop, eventually everything returns to your original size.

Try not to focus too much on the stretched penis length. My own surgeon refuses to even do it. Yes it is an indicator of what you might achieve, but the only measurement that matters is when your surgeon dilates you in the OR and figures out the maximum size implant that your anatomy can safely accept. It's correct that they can't install an implant that's too big, it would cause major problems that you don't want. You are already doing the prep for both prostate and implant procedures through routine VED therapy. You will have to stop for a few weeks after your prostatectomy as they have to cut then re-join the urethra. I recommend that you resume once your doc says it's safe to do so. It can help recover any size loss post RALP, as well as stretch the penile tissue in preparation for an implant.

I pumped aggressively every day in the lead up to my implant surgery. I was naturally 6.5 inches erect pre prostatectomy and a bit under 5 inches girth.
Immediately post implant op my dick was much shorter (about 4.75 inches), left partly erect for 6 weeks, black and bruised, scrotum swollen to the size of a grapefruit. Once I started cycling the implant after 6 weeks I soon started to regain my original size. Eventually achieved slightly over 6.5 inches erect after many months, girth has stayed the same as I've always been. I'm now a significant "shower" with a flaccid over 5 inches (used to be about 3 inches historically). You will also likely find that you become a shower, like the majority of us post implant.

Set your ambitions around your best natural size in the past, not what the VED can do. If you were naturally always 6 inches erect it isn't unthinkable that you might get that back over time with a competent surgeon, then healing and cycling. It can be a long journey for some, so please follow all your own doc's protocols and don't give up.

In any case from my experience if a doctor isn't confident that he can restore the full length better to find another more experienced doctor that will stretch as much as possibile

What the doctor told you is mostly true but not the whole story. When an implant is placed, the size used is based on the *internal* measurement after dilating the cavernosal spaces. Stretched flaccid length is an analog to the length of implant installed after dilation, but it’s only a rough estimate.

You’ll get an implant based on the stretched internal length of the cavernosa. Many of us (me included), had a significantly shorter stretched flaccid length than the actual length of the implanted penis.

The best thing you can do to maximize your length is to use an experienced, high volume surgeon. Also using the VED right up until surgery will help. I even used the VED the morning of my surgery.

None of us think that our flaccid stretch is really indicative of our true size, but the vast majority of us end up with implants that are the correct size and preserve our normal size.

Here’s my take on the stretched flaccid test. The doctor is going to always underestimate. They do not want to give you an unrealistic expectation, but would rather it be a better outcome than a worse outcome based on what they tell you that you’re stretched flaccid test. When I had my stretched flaccid test, the doctor said I would be 4 1/2 inches. I’m currently about 11 weeks postop. I’ve been cycling for a little over two weeks and I am at 6 inches fully pumped.

Agreed with previous comments. The method of sizing you for an implant on the operating table does not change from doctor to doctor, though a small number of inexperienced surgeons might size you incorrectly.

But, for some reason the proxy measurement of the stretch test doesn’t adequately indicate what your erect length will be post implant.

For example, in the study linked below they measured men twice before the implant operation, once using a stretch test, and once using an injection induced erection. The data indicates that the stretch test on average resulted in a penis measurement that was two centimeters shorter than the injection measurement. Why? Who knows. Maybe docs are timid about yanking on your cock while you are awake, but less so when you are under and dilated. Or it could just be that the internal measurement made doesn’t translate to what a non-erect stretched dick looks like.

Regardless, after two years of cycling virtually all participants in the study were back to their injection-induced length. So in a manner of speaking you could say they gained, on average, about 2cm on their stretch test length.

Also agreed that a VED induced erection inside the tube is entirely unrealistic… that’s just from massive engorgement, not from the cavernosa.

Overall, don’t sweat it. You are going to have a super reliable dick available on demand for as long as you want. If you are seeing an experienced implanter, it will be the longest implant your dick is capable of handling. Good luck!

1st of all good luck with your cancer op. Like its said before VED is no real measurement. I am about the same size like you. The other day I was on a nude beach, 1st time Ifelt confident and relaxed. I knew when I have my old size back and my dick is hard like before ED hit there will be no problem to satisfy women again. And that's the case now. 6.3+ 4.5 is sufficient as long its hard as a rock. Believe me. All the best.

First things first. Get rid of the cancer and heal.

It’s good you have been using a VED to regain/maintain size. As soon as you have healed enough from prostatectomy that your surgeon gives the OK, get back to using it regularly. It will help.

DO NOT have the surgeon you talked to do your implant. They already told you, you’ll have an unsatisfactory result. Believe them.

Instead, research who the highest volume implant surgeon you can reach and have them do it instead. Keep using the VED and a great high volume surgeon will get you the maximum size your body can accommodate. It won’t be the size you are in the VED, but it will be good.

The common experience of guys who were growers is that post implant they are showers. And they generally seem pleased with their erect/inflated length after a few months the of cycling the implant.

If all the guys who were originally growers were condemned to having short implanted penises you’d hear it here.

Hang in there. It’s going to be great. Get through surgery and if you need an implant you’ll be happy in the end.

Niculus, I was freaked out a little too when my doc pulled on my shriveled up dick and said that was going to be my size.
I was a big grower too.

I used a VED also before my implant surgery and I was getting around the same size as you too with the device. Inside the VED my dick would get big and plump like an Oscar Meyer Weiner! lol

I'm happy with my implant, my Army Major Doctor did a great job, and after healing up and doing all my cycling exercises, I'm at 6.75 to 7 inches.

I have a AMS LGX 18cm with 3cm rear tips, I've had it for 5 years now and I'm very happy.

The doctors won't know what your true size is until he gets in and dilates your inside and takes measurements, for your distal and proximal measurements.
