Cylinder placement

I had my titan implant on June 28, 2013. I grabbed my pump everyday 3 times like doc said and my pump moved all the way up and is at the base of my shaft it hurts sometimes to deflate it but that is not the problem. My problem is that the surgeon who does the surfer multiple times per week had trouble with my erection chambers and was not able to get the cylinders into the head of my penis and i am having trouble getting my penis to penetrate and my head is floppy. I also pump it everyday and am about 4 inches at most and was about 8 inches before the surgery. My doc told me i should get back to about 8 inches again. Anyone else have their surgeon not put the cylinders all the way to the head of their penis?
