Something loose like sweats. You don't know how much swelling or bandages you'll have so you want to make room. Plus you might not want anything that binds or rubs. But, you might also be more comfortable if your penis and/or scrotum have good support. Or not, but better to be prepared. Some docs will give you some support right away (eg. I was strapped into a jock strap on the operating table that I was instructed to wear 24/7 for 10 days). But if they don't supply support you might (or might not) want something like a jock strap or support briefs. Some guys really liked jockey pouch briefs.

Also, if you have day surgery as opposed to an overnight stay, you most likely will come home with a catheter in place. You will want something baggy enough to accommodate that. But your doc can tell you if you will likely have a catheter at home.

For me, soft thick bandages were great. Be nice to the nurses and they can probably set you up with something customized to your liking.
