Paying out of pocket or don't have insurance? Read this!

Hey guys,
If you're paying out of pocket or don't have insurance, AMS is running a brand new co-pay assistance program designed to help potential implant patients out. If you qualify, you are able to receive one or both of the following options:

Co-pay assistance grant: $2,500 deducted from final out of pocket expenses.

Co-Pay assistance smoothing. Help pay up to $17,500 towards out of pocket expenses. Amount is repaid in interest free monthly installments for 24 months.

Some terms and conditions:
Must repay all assistance amounts (grant excluded)
Not available for Medicaid and Medicare patients
Only available in the USA
Not available to residents of Minnesota, Missouri, Michigan, Massachusetts, Vermont and Rhode Island.
Offer Expires September 15, 2015

Same state exclusions again. When I applied I was told to reapply when I move out of the state of Missouri. This bites man. d
