length and girth
I did a brief search and did not find an answer so here we go. I have yet to inflate mine (2 more weeks(. I was to the point where nothing worked. Pills worked for a couple years then injections tried pump nothing! so I had no other alternative so I did not ask enough questions. I was better than average before the operation. My X could not take it all. I do not care if I lose some size as it did not work for the last few years so I had nothing! I see many statements about doctors measuring I never got measured never asked what size I was. I was very small limp but grew to over 8" with injections and a little better before the problem. Did I screw up or does the doctor use that stretch method while I was knocked out?
Re: length and girth
My doctor never actually measured me either, he did pull on it, looked at it and the scrotum and said,"Yeah, you'll do fine." The important measurements are done in the OR while you're out. That's when they use the dialators to open the space in the corpora and insert tools to measure the distal & proximal length. When they measure the distal length, they usually will exert a pretty powerful stretch to get as large a measurement as possible.

^ What Don said. The stretch test is just an estimation - the real measuring happens during the procedure. The actual length of the cylinders depends on your anatomy, and the internal measurements are what determines the size used.

Thanks guys I am not worried about being exactly 8" but 4" would suck lol
Re: length and girth
I had an appointment with one of the highest volume surgeons in the penile implant industry. Since I'm very concerned about losing size, we talked in detail on this subject.
The primary and most important measurements are made in the operation room. He showed me some picture of how it's done.
Measurements are also taken pre-surgery. This particular surgeon did a stretched measurement, and an erect measurement (induced by an injecton - prostaglandin). He said these two pre-surgery measurements are not as important as the actual measurements taken in the operating room, but they do help with the sizing of the implant.
Basically when you're under the knife, they make the incision in the scrotal area, and they measure the length/capacity of your penis by putting a cylinder shaped device in your penis via the incision - they put this in the same place/cavity they'll be putting the implants later. This goes all the way to the end of your penis and shows the measured length in centimeters. So basically a cylindrical shaped ruler. That's the first measurement. The second measurement is done from the incision as well, but goes into a different direction towards your body. The two numbers are added up.
So for example if your penis capacity is 18 cm long via the device I mentioned, and the distance to your body is 4 cm, then these two numbers are added and the surgeon will use a 22 cm implant.

How big is the implant, and what's your penile length post-op?

I pumped to 5 pumps today and I would say 5 inches did not measure no pain so may try more pumps later
