Which is better...ice pack or heating pad? Or both?

Heating pad was much more comfortable for me.

Cold is to reduce swelling. After surgery you should only use ice. Typical timeframe is 72 hours after. Then heat, as long as swelling has gone down.
Heat too early can cause extra bleeding and bruising. Also heat will accelerate the growth of bacteria.
Alternating cold and heat, after swelling is absent, can help force blood out (cold) then bring fresh blood and oxygen in (heat).

My directions were only ice until there was no sign of swelling. After you are over that hurdle the heat helps loosen things up. That is helpful distinguish the pump from your balls, which at least for me felt like one solid mass after surgery. Heat, like a warm bath can also make the actual
process of pumping much easier because it softens the hardness of the new bulb.

After about 5 weeks, swelling went pretty much away but still sore, hot bath helped out greatly
