When does Priapism become dangerous?

Unfortunately I've had a number of erections lasting 6 or so hours with Trimix. My penis has never felt cold when this happens, indicating that there is still some blood flow happening.

At what point is Priapism actually dangerous? I figure that the four hour mark assumes that the patient will go to the hospital at that point and it may take a couple of hours after that for treatment.

I was a paramedic for 15 years... I can tell you that according to the general medical community, at the 4 hour mark, you are considered to have a priapism. At that point, you need to seek medical attention.

My doctor felt 6 hours, although painful, would only cause temporary bruising and not permanent damage. I downloaded a medical paper studying this and felt better. Permanent damage happened but typically after much longer times, like 24 hours

From everything I've read: 4hrs is the safety mark to get medical attention for an ischemic priapism.
At 6 hrs the blood can become acidic and start causing damage.

After 24 hours you have a 0%-40% Chance of recovery. Less than 24 hours you have 78%-100% chance of recovery.

Out of medical curiosity, what exactly are they doing to treat priapism? My understanding is that the doctor will "drain" blood from the penis but does anyone know the technical details behind this? Not that I have plans to do this myself, understand, but to actually know the process would be fascinating.

I would guess that a larger gauge needle, like a 25 gauge (?) would be used so that blood would flow into it more easily than a smaller gauge needle. I'm sure that you can't pull any blood through an insulin syringe...or if you can, it's an extremely slow process. Would the doctor then drain a certain amount of CCs or milliliters from the penis? Would the doctor drain blood from just one side or do both sides equally? Anyone know answers to these questions?

Needle are bigger as the gauge gets smaller... A 25 is very tiny. Blood is usually drawn from at least an 18 and in many cases, a 16. NOT COMFORTABLE in the penis.

My doc said to be concerned and head to the ER if you have hard wood for over 6 hours. He said dont sweat a partial as you would have blood flow.

I had a urologist tell me they can start to see tissue damage after the four hour mark. One time I went to the four hour mark and was about to drive to the ER when it started going down. I wouldn't mess around with a priapism.
