Results with Bimix (30:1)
My urologist has given me an r/x for Bimix and so far results are less than great. I tried 0.2mL and now 0.4mL and both times there is good enlargement, but it lasts only 5 minutes or less and never gets rigid. He said we can try Trimix, but I'm concerned it won't be much better. Anyone have similar experience with Bimix, followed by good results with Trimix?
Married for 49 years, 71 year old in Indiana. ED pills no longer effective. No Surgeries. Now on 0.8mL-1mL BiMix (Papaverine HCl 30mg/mL, Phentolamine Mesylate 1mg/mL).

Welcome to the forum.
I'm not a medical professional of any kind. Anything I post is based on my own experiences or readings, at best, and on hallucinations or delusions at worst. Any advice found on an internet forum is very likely worth what you paid for it. Always ask your doctor and follow his/her advice or suggestions.
1) Do you know what's in your bi-mix?
2) Is bi-mix what you started on?
3) Have you been tested for a venous leak?
4) Were the oral medications ineffective?
As for injections I think it's a bit unusual to start on bi-mix. Often the doctor will start you out on some form of Alprostadil also known as Prostaglandin E-1 or more commonly PGE-1. All the same stuff. Commonly it's found in the name brands of Edex or Caverject in proprietary self contained injection kits and is expensive. It can also be found at some compounding pharmacies in a generic form sometimes called Mono-mix. It's much cheaper than the proprietary brands and priced comparably to tri-mix. It is generally considered the most effective ingredient of Tri-mix but is usually missing from Bi-mix. It is my possibly incorrect understanding that bi-mix is usually prescribed when the patient can't tolerate the PGE-1 ingredient in Tri-mix.
So yes, I'd give the Mono-mix or the Tri-mix a try. Either might do the trick. The alprostadil does produce an ache or pain in some men that will often disappear or moderate with continued use, but not always. That's generally when men end up on Bi-mix.
If you have a bad venous leak the injection meds won't work as well or at all. Sometimes adding some sort of novelty cock ring added to the injections can help in those cases or even a medical grade constriction ring. However if a particularly tight ring is used there will be time limits on it's on it's use. Otherwise, injections won't be the answer for you. If the oral meds also haven't worked that currently leaves you with the options of using a VED (Vacuum Erection Device) with constriction rings or ultimately an implant. Some men are satisfied with the VED and constriction rings. The majority of men with implants seem satisfied with the results.
I hope that was helpful.

Thanks, that was helpful.
Yes, Bimix is what he started me on. It is Papaverine HCl 30mg/mL, Phentolamine Mesylate 1mg/mL.
I have not been tested for a venous leak, but like you, I suspect something like that.
The only oral medication that had ANY effect was Cialis, but that was insufficient for penetration.

I have had continued success with Trimix since the very first injection.
Born 1948, wed 1969. BPH & Type II Diabetes at age 35. TURP-2002; ED even before that--diabetes. Cardiac valve surgery: 2007 & 2019. Poor results with pills. Started trimix injections in Nov, 2010. Great results from the very beginning.
Re: Results with Bimix (30:1)
My doc started me on Bimix. His thoughts seemed to be that if Bimix worked (it did) why move on to Trimix and risk potential pain. I think he said 20% of guys suffer pain using Trimix. No pain with Bimix and it does the job.
Retired. R.P. 2016. Bilateral nerve sparing surgery. Now use .15cc of Bimix twice weekly & anorgasmia, moderately incontinent. Wife no longer interested so go solo with Electro-stim using Erostek ET-312. Now am Type 2 diabetic.
Re: Results with Bimix (30:1)
I was on Trimix first. Doc gave me 30 units. Damn near killed me with pain and lasted 4 hrs. I tried lower doses but still pain of 20 on a scale of 1-10. Got a script for bi mix 30-2. 40 units didn't give me a useful hard on. I would put 2-3 units in the syringe of the old trimix formula 8.5- 30-1 and 20 . Then fill the syringe to 29 units total using the bi mix 30-2. Moderate pain with a hard on that lasted 3 hrs. The pain is from allergy the PGE-1 in trimix. PGE-1 is the #1 hardening agent. The other 2 just keep it there. 30% of guys are allergic to it.

Interesting comment in your profile Reggieman - 0.15cc BiMix. I find that 0.4 is not effective enough (less than a 3-5 minute useful zone). How long does your "useful" erection last at 0.15 ml?
With TriMix, I hear that the issue is a possible allergy to the main ingredient. I suppose the only way to know is to try it?
Thanks for all the comments.

Peterknight:
15 units will give a useful erection of 1.5 to 2 hours before it has deflated too much. I usually use 12 units when having sex (or trying to) with my wife. I can't orgasm with her so when she is satisfied I turn to electro-stim in an attempt to reach something like an extremely weak orgasm. I'm usually successful about 49% of the time. I can't get more than 90% of a normal erection due to damage to my penis.
If I am just trying to masturbate with the estim then I use 10 units that gives a good erection for an hour. However, even as the Bimix wears off the stimulation from the estim device will cause a full erection. If I have no Bimix in me no amount of electro-stimulation will generate an erection. Other than no erection everything else is the same during these sessions.
It takes 4 hours for 10 units to totally wear off. It's nice to have a penis that looks normal for 4 hours. When taking a shower in the morning and looking down at my penis I think how pathetic it looks. It use to have such a life of its own. Now it's just dead looking. LOL
The doc said I need injections twice a week and I follow his instructions. Sometimes I will use the estim device without an injection. I get no erection at all.
Due to orgasmia my estim sessions last 45 minutes to 2 hours (usually the longer period) with no guarantees of success. It feels good even without an orgasm but it is difficult to coordinate the correct amount of mental and physical stimulation to create something like a weak orgasm...it all has to come together at the exact same moment. It's work. LOL If I'm not careful then my penis goes numb from over stimulation.
I use porn and now literotica to get the mental part going. After 2+ years since my RP I'm getting burned out on porn (most of it is so fake and stupid). The kind of porn that appeals to be is very limited (non-professionals that are actually having a good time just being themselves and who do the things I like to do...sort of vanilla. Insert another LOL here.
Having never tried Trimix I don't know how I would react. I don't think I would get any harder as I appear to have suffered some smooth muscle damage after the RP. A portion of the left side of my penis does not inflate very well which causes a lean to "port". The glans does not inflate unless I use electro-stim. I was on Cialis after the RP but it did nothing for the glans.
I don't recall the surgeon telling me about the potential for orgasmia so it came as a very unpleasant surprise and has a negative effect when it comes to sex with my wife. I still like the physical contact with her even without the ability to orgasm. I like the closeness and all that goes with sex with a woman.
It's taken us since June 2016 (RP) to iron out what our sex life is to be. She has some odd ideas about sex which I have posted about before. She has finally come around to be more cooperative and less begrudgingly (only took 19 years) about sex with me. I think it finally dawned on her that life with a happier husband was more pleasant than life with one who was always angry even when he didn't show it.
I seem to be droning on.

Reggieman,
I'm sorry you're having such difficulties. Unfortunately I can relate to more of it than I'd like.

Re: Results with Bimix (30:1)
Reggieman,
Thanks for information.
I am thankful that I only have ED due to poor blood flow (at least, as far as I know). My prostate is enlarged but not seriously interfering with urine flow, and I have to pee at night 1-2 times.
I find that in the mornings, I can get an erection with only masterbation that's as good as 0.2mL BiMix injection - but doesn't last long. My BP is lowest first thing and rises throughout the day; I think that's a factor.
I guess I'll have my urologist try me on TriMix because I don't think he'll suggest going more than 0.4mL on the BiMix. Does anyone else use as much as 0.6mL BiMix? Or can it be made stronger than 30:1?
