I am pretty sure ,inflated.
The interior measurement of patient’s tunica is completely stretched out. So the inflated implant length should be matched to that dimension

Inflated and adapted to your corporal mesureament. You must know that corporae end just before the glans. See anatomy picture to see it better. So inflated, your shaft will be hard until mid glans, the very end is soft. If you are aroused the spongiosum engorgement make the glans grows.
If you have soft erection before implant you ll have on top of your implant erection.

I thought If the Doc gives you a 20cm titan for example with no Rtes then depending on how deep your crus is a portion of that 20cm is buried inside (non useable for sex) plus 5cm of that titan is fixed and does not expand so you really only have say 15cm out of 20 cm that inflates. Please correct me if I’m wrong someone. So to recap... 5 out 20cm is fixed and doesn’t not expand and depending on depth of crus you could lose another 1-2 cm of the inflatable portion inside the body which is not useable for penetration. So you may end up with anywhere from say 13-15cm that are actually useable for penetration???

I think the whole cylinder expands/hardens, but either way your "usable" measurement sounds about right.
56. RARP for prostate cancer in Feb. 17, then no-go with pills, injections, vacuum, so here I am. Titan 20cm by Dr. Eid on 8/6/19, cycling on Day 4, stitches out Day 14, sex with a grateful wife on Day 20. And 21, 22, 23, etc.

The length is the inflated length.
Yes, both brands do have a 4.5-5.0 cm fixed part in the back which cannot be inflated.
Yes, part of the implant goes into the crus and cannot be used for penetration.
Rule of thumb is one third of your implant is inside your body and two thirds in your dick.
Example, 22 cm implant, no RTEs
Inflatable length: 22-5=17 cm
One third inside the body: 22/3=7.33 cm
Inside body is 5 cm fixed uninflatable part + 7.33-5.0 cm = 2.33 cm inflatable part
In dick is 22-7.33=14.67 cm, all inflatable.
If your doc has done a good job, you will have your implant up till 1.5 cm from your glans end.
Meaning your dick will be 14.67+1.5=16.17 cm.
Example, 18 cm + 4 cm RTEs
Total implant length is still 22 cm as above
Inflatable length: 18-5=13 cm
One third inside the body: 22/3=7.33
Implant has in total 5+4 = 9 cm uninflatable part
One third of total implant length is inside body, i.e. 7.33 cm
This means that of the 9 cm uninflatable length, only 7.33 can be in the body.
So, we have 9-7.33 = 1.67 cm of uninflatable fixed part in the dick.
Of course, we still have a total length of 22 cm, and 22*2/3=14.67 cm in your dick.
But of those 14.67, only 18-5=13 cm will be inflatable.
If the second case is happening, I guess this will be unpleasant since you will have fixed uninflatable parts in your dick.
Hopefully the doc wouldn't have chosen the setup of 4 cm RTEs on this hypothetical patient.
The crus depth can differ of course from man to man, and if this guy had a deeper crus, all his RTEs would have been inside his body.

With a good surgeon you won t loose lenght, maybe 1 cm (1/4 inch). Stretching your penis is a good indicator.
