I think many of us with implants now wish we could have skipped past everything else we tried along the way. Unfortunately, I think most reputable doctors, as well as insurance providers, want to make sure those other methods like pills and injections do not work before they agree that an implant is the next step.
I am 64 and had ED from a VL. Implanted by Dr. Ronald Anglade in Atlanta on 9/18/17. I have an AMS700LGX 21 cm via a Penoscrotal incision. Very happy with results. 6" soft and 6 3/4” x 5 5/8” hard.

Looking back, I would say I should have got the implant when I was 30! Lost size and frustration for 35 years. I went through pills and injections I’m sorry I did. Yes the intelligent way is to use the least invasive methods first. I just found them incredibly frustrating.

While the pills worked, I would prefer them. Getting an erection in response to my lover's attentions is SO MUCH BETTER than pumping up manually.

Besides, pills and injections do not destroy the potential for new non-invasive ways to restore one's erectile ability. (Hormone therapy, stem cell therapy, etc).

The surgical procedure to implant any type of penile prosthesis forever destroys completely (in 99.9% of cases) all hope of any other solution or treatment. It is an irreversible step.

Having said that, I skipped suppositories and injections and went straight from oral medications to an implant (my hormone levels tested normal, so no hope there).

I never had a single moment doubt once my researching was done.

Same situation for me.

Now i have to choose between injections or implant. Pills works little bit but not enough for having sex.

I took the natural progression. Pills then injections (after PC surgery) then implanted after a second round of cancer required radiation and the shots quit working to my satisfaction. As my handle suggests, I took over 1000 injections with no damage and no scarring. I believe that most can have good results if they do the shots right. My uro gave me needles that were way too large. Even the largest guys can make a #31 x 5/16 needle work.

My regrets are that I didn't realize the atrophy that was reducing my size, and my uro kept telling me I would have no size loss. Had I understood what was going on I would have used a VED constantly up until my implant. I am average now but miss my 7 x 6 1/2 size. Now 5 1/2 x 5 1/2. I love my implant but i had a good sex life with the other methods, and other than working on size I probably wouldn't change anything. Had I been implanted in my 60's there would be a good chance of needing a replacement down the road. I am hoping that this one gets me out of here :)

That's a very good question.
My ED was there all the time to some degree, even though it got worse slowly year by year.
In the beginninng, Viagra did a good job. But as many here say, they slowly stopped working. Actually, in my case, I don't think they stopped working. It was just my ED getting worse, and eventually their effect wasn't enough anymore. At least not consistently.

I never did the injections, and I am glad I didn't.

So would I prefer pills today if I knew they worked?
First of all, that question must be defined. What is "working"?
Sure, if I KNEW that I could take one Viagra, and then anytime form 30 minutes till 4 hours afterwards, I could 100% trust that I got a great erection which lasted tll I came - then yes, I think I would prefer that.
But with how many of us does that really happen? If you are one of them, then congrats.
In my case, even in the beginning, pills made a huge improvement, but they did not achieve what I said above. !00% certainty of an erection lasting as long as it should.
Pills took my chance of getting an erection from 80% to 95%. They took my hardness from 80% to 95%. They took my chance of lasting 20 minutes from 50% to 80%.
But that is still not enough, or it wasn't for me. There were still failures now and then. There was still this feeling of having ED. Of knowing every single time that it might go to hell.

An implant comes with its own drawbacks, but one thing is for sure. You are going to get hard and that hardness is going to last till you decide to deflate your dick.
That is the huge advantage, which in my case makes it the best choice for me.

But again, if - and this is a huge if - pills really worked, meaning completely wiped out my ED and never failed, then yes, I would probably rather be on the pills.
But ask yourself that question, and answer honestly - do the pills really work, as in completely wipes out your ED for a few hours?

Pills and injections RARELY worked for me. I finally gave up trying to have sex outside of some self-service which required, pills, injections, and porn. Truth be told the surgical options scared the crap out of me and I envisioned a very painful recovery. I finally got my implant 5 months ago. Recovery was painless. I wish that I would have had it done day one when my cock doc diagnosed my leaky bucket problem using ultrasound. It's still a scary procedure so find a great implant surgeon, but if pills and shots don't get it done at least 50% of the time than get the implant now. I have MS and my health has deteriorated much in the 10 years that I waited. I missed many very exciting opportunities. :(
60 years old. MS and Severe ED 10+ years. Pills and injections never worked well. Implanted 5/16/2019 by Dr. Sadeghi. Titan 20cm + 2cm RTE.
