the updated "list" of high volume implant surgeons

I was looking through literature and came across a paper written in March 2017 which was a study from 26 centers. This is an important list, since it represents "the club" of experts in penile implant surgery.

Many of the usual franktalk surgeons are on the list : (Eid/perito/garber/Ralph/).

Abstract
INTRODUCTION:
Penile prosthesis infections remain challenging despite advancements in surgical technique, device improvements, and adoption of antibiotic prophylaxis guidelines.
AIM:
To investigate penile prosthesis infection microbiology to consider which changes in practice could decrease infection rates, to evaluate current antibiotic prophylaxis guidelines, and to develop a proposed algorithm for penile prosthesis infections.
METHODS:
This retrospective institutional review board-exempt multi-institutional study from 25 centers reviewed intraoperative cultures obtained at explantation or Mulcahy salvage of infected three-piece inflatable penile prostheses (IPPs). Antibiotic usage was recorded at implantation, admission for infection, and explantation or salvage surgery. Cultures were obtained from purulent material in the implant space and from the biofilm on the device.

This is one of the best sources I've seen so far - thanks Tangerine.

At the risk of stating the obvious - things change. For example, the star impant surgeon at SIU (which is on the list) was Tobias Kohler ... who has moved and currently practicing at Mayo Clinic in Rochester MN (which is not on the list). So not meant as a criticism, just a reminder to the team to make sure you cross check this list against the surgeon you actually want doing your work ...

Tangerine, not to pick apart list, but title is Surgeons, not hospitals.
BIG difference between hospitals and surgeons, and I am living proof!
Chesapeake Urology Dr #1 fucked up placement.
3 mos in, Dr #3, Dr. Kramer (high volume Dr), also with Chesapeake Urology, salvaging fuckup of Dr. #1
Dr #1, 6-10 years experience. Dr. #3, Dr. Kramer, over 21 years experience.
Just saying...patient beware and also check out Dr.

Don't know if Admin can change title, but I just wanted to point that out for all that has been posted so far.

yes, thank you for mentioning that issue regarding hospital versus surgeon. Clearly, whether or not your doctor was invited to be coauthor on this single paper should not be consdiered a significant indicator regarding overall quality.

As I took a look at the list, you are correct -- these are considered centers of high volume for implant surgery, and in the case where there was more than one penile implant surgeon, they (to be considerate) listed all the major surgeons doing implants at that center (for example Boston University lists Dr Phillips and Dr Carrasquillo and dr Thornton).

So, this list is not the end all be all of lists. I found the artcle since someone was asking about Dr carrion and I noticed he is on this list. The paper was a multicenter trial, and the authors were selected by the lead author of the paper since this group was felt to be a good sample of high volume surgeons.

Please notice that a majority are in academic university teaching hospitals, which some would say is a good thing or a bad thing. The list is from 2017, so it is 3 years old. The list represents the "academic club" so some younger really talented phenomenal surgeons did not get invited or listed as authors (eg: Dr Kramer from baltimore and Dr Karpman from California)

So, yes, the list is very imperfect !

This list missed all of the west coast. Dr Karpman , San Jose area, Dr LoDoyle at USC Keck in LA. Lo Doyle assisted Dr Boyd with 300 and was primary on 80 in the last 2 years. Dr LoDoyle is currently doing 3-4 a week. Dr Boyd was the go to guy in LA for 30 years. He is not completely retired if you want to ask for him. Lots of implants done in the Phoenix area but I don't have names.

As before, the letter after the surgeon's name is then referenced to the hospital where he came from. Very happy to see that Dr Karpman is on the list. This paper was published in 2015.

Hahnemann University Hospital, Philadelphia, PA, USA. Represented 2 surgeon , and this hospital went out of biz in the last 6 months.

Bruce Garber in Philadelphia is no longer in practice.

well it is March 2022, and names have been added and deleted from the list.

I will quote a nice summary from another franktalk brother who states:

That sucks, bro. I hope you can get some answers. FWIW, I'm six months out with my Perito implant and have had none of the issues described above. I also don't hear about many patients of Eid, Hakky, Clavell, or Karpman having many issues, either. That seems like the A-list at this point. I know Kramer is considered elite, and maybe he is, but most of the stuff I've read from his implantees has been negative.


Here are my thoughts:

I have direct personal patient experience with Eid and Karpman (I live on west coast, so Karpman is my local implant surgeon that I see). Those two are , in my opinion, rock solid, very talented, totally professional formidable doctors.

From my reading of things here on franktalk, I agree that clavell and hakky seem to be A listers as well (though I have no experience with them directly)


the other names on the paper which seem to be discussed here on franktalk as beeing good are:

Perito , Ralph (United kingdom), Köhler , Carrion , Eid


so, it seems that the "short list" of top surgeons in USA are:

Eid
Hakky.
Karpman
Clavell
Perito
Kohler
Lodoyle


Publishing the list of top surgeons is fraught with inaccuracy; but the above names seem to be associated with happy patients. That is not to say that those are the only A-list surgeons, I am sure there are 6 more that are not on the above list

Tangerine, I live here in Northern California and I have an appointment with Dr. Karpman on the 24th, what is general lead time from your first visit till he could do an implant, I am wanting to get it done soon so as not to interfere with my summer plans. Is he booked out a long time?
