Do you think this issue could have been avoided by having the corporotomy further back, more proximal to the body?
That may keep the tubing away from the ring.

I assume this could only be achieved by minimising (or ideally eliminating) the use of RTE's, assuming you have any.
Also, I assume the tubing rubs against the ring, but is not under the ring. Is that right?

There is a thread on here about the use of rings with an implant, and the differing results you can have depending on if you received a penile scrotal or infrapubic installation.

I'm apparently a unique case. I had severe fibrosis in the proximal corpora due to Peyronie's. I had a 2cc (2x2x2 cm) calcified or fibrous plaque at the right dorsal base that I could palpate. When the doctor came into my hospital room, he said he ran into some challenges (either he didn't elaborate or I don't remember). I read that to mean that he couldn't fully access the proximal corpora due to fibrosis. In the last appointment (consultation) before the surgery, he said he should be able to get through it okay and would give me whatever was possible.

I have had one month to "do the math" on my situation although I've only started thinking about things since after my 2 week followup. Here's my thoughts:

I have at or very near 5" from bone to tip of glans. May be more like 4.75" or slightly more.
It feels like there is about an eighth of an inch of glans over the distal tips.
I was implanted with a 15cm X 12mm AMS 700CX. He didn't install any RTEs (Rear Tip Extenders).

15cm = 5.9"

4.75" length calculations:
4.75" (penile length) - 0.125" (glans depth) = 4.625" (distal length of cylinder).
5.9" (tip-to-tip cylinder length) - 4.625" (distal cylinder length) = 1.275" of proximal penetration of the rear tips into the corpora

5.00" length calculations:
5.0" (penile length) - 0.125" (glans depth) = 4.875" (distal cylinder length)
5.9" (tip-to-tip cylinder length) - 4.875" (distal cylinder length) = 1.025" of proximal penetration of the rear tips into the corpora

Typical ratio of distal to proximal corpora cavernosa is 2:1. For 5" distal, that would be 2.5" proximal on average. I only have 1.025" to 1.275" of penetration into the proximal corpora based on the numbers.

That's a real bugger. I did have a search of your history after my last post to find you had been in the wars with all this.

Did he say why he couldn't get through the plaque? What stops him hacking through? That question probably tells you how little I know.
So I guess that's where the corporotomy was always going to be for you, no choice. :(

Do you feel that the implant is anchored deep enough to avoid a hinge effect?

Yes i have used it and the doctor is right. It burns and did not help anyway..forget it.

I have a 3 month follow-up coming in February and I'm going to try to get all my questions asked. I thought he would dig through the fibrosis as I've seem references to using blunt surgical scissors to spread out and dialate the scarring. Not sure why he was unsuccessful but if I was to guess, I'd say it was probably not so much fibrous but rather calcified so it wouldn't give when he tried to spread it.

As for depth, it's probably barely deep enough but maybe not. I did pump to max and test it's rigidity against a pelvic thrust by pushing it into my groin... It doesn't give and slip further back, so it seems well anchored, but there is some hinging (although it doesn't collapse at the base as it did before). It's not rigid from my body out. It bounces around a bit. When inflated, if I'm on my back, it's leaning a couple of inches off my belly. If I'm slightly reclined (still on my back), it sticks straight up. If I lay on my side in either direction, it hangs down a little bit below three o'clock. If I'm standing it's also below three o'clock (3:30-4:00). Also at day 35, when pumped to the max my corporotomies ache in pain after a few minutes and the tubing that exits them is rigid, rock hard.

I may investigate revision surgery with a high-volume surgeon in my area of I have issues when I start trying to use things.

I was implanted with a ams lgx on 2-26-14. Been using cialis 20mg twice a week with no problems at all. The cialis increases both the size of the glands and the girth.
70yrs. Implant AMS LGX 18cm 4cm tips. 2-26-14. Implant installed because of fracture.

I had an odd "problem" (pain) today when I got up and I was initially a bit concerned. I traced it to my cylinder tips causing the pain in the glans. I thought that was kind of weird, considering I've been post-op for 49 days and this was the first time I felt the pain without being pumped up.

Secondly, I noticed the dorsal skin that adjoins the glans corona, was stretched out. I'm referring to the skin on top of my penis that connects to the ring around the head. Instead of the corona (ring around the head) being defined all the way around, the ring disappears at the top of my penis as the glans and dorsal skin appears as one without any division. I could pull the skin toward the head and the corona was visibly defined again.

I thought maybe I left my cylinders partially inflated so I pumped once or twice then deflated but after about 15-20 minutes of deflation, the coronal ridge disappeared again and after about an hour the twinges from the tips resumed.

**THEN I HAD AN EPIPHANY!!** Last night I took my first quartered Cialis 20 mg (5 mg daily dose). I decided to try taking Cialis as "daily dose" to see if it would improve the engorgement of my glans.

Bottom line is that I'm chubbing and my glans is now more engorged than it has been recently (it's spongy like modeling clay instead of totally soft and floppy). I figure it will take a few days to get used to the new chub and the glans will start to become more comfortable with the tips.

While I was on Bimix, Dr. recommended the daily 5mg Cialis for my blood pressure too. When I going to have sex, go ahead and take the 25mg Cialis. Nothing to write home about. Nothing extraordinary came about, so I just kept taking the 5mg Cialis, even today, after my malleable implant.

I have found that after a short acclimatation period to Cialis 5mg daily dose, the pain or discomfort has subsided and I no longer have a totally flaccid glans anymore. It's almost always at least somewhat spongy now, instead of the feel of a wet rag. It doesn't get rock hard during arousal but both the glans and urethral area do become more tumescent and more rigid.

I have used the Cialis 5mg regimen that was suggested by my doctor to help firm up the glans. Eventho I have the implant i have been using the VED. That helped increase girth and length since the glans does get more engorged with blood.

If one pumps up the implant it appears to be oval shape and not round. With arousal it starts to round out like it should be and it gets that way since more blood gets to the glans. So the Cialis helps a lot i think, for me. I have also tried using rings too...and that is good too. The glans really gets big and wife loves it even more...

My surgeon told me the VED is okay to use if i max the implant first. Have not had any trouble so far using it for therapy too.
