I have had both peri-scrotal and infra-pubic implants. My first implant (peri-scrotal and botched by my local urologist) was "fixed" a year later by Dr Perito. I truly did not find him cocky nor arrogant. And....he REALLY knows his stuff!!! He did run into at least one unexpected problem on my revision and dealt with it like the great surgeon he is! Three years later, my implant "went flat". With my "second revision" - "third implant" - "fourth dick", Dr. Perito encountered another problem. My current implant works very well, although I have lost a little more "sensation". I have great fun but rarely orgasm.

In essence, I went to Dr Perito with a very painful botched implant and a "cold head". Dr. Perito fixed what he could fix.

I found Dr Perito to be knowledgeable and interested in my problem. I also found him to be quite funny.

Dr. Perito is "My Hero"!!!

I am glad I put my trust in him. I wish he'd done my first implant (my bad.....!!!!)

I really don't understand why you are so obsessed with doing it infa-pubic?
I can't find the link to the article now, but I have linked before to an article basically saying the only advantage with the infra-pubic is easier reservoir placement, which is good if you have pre-existing conditions making reservoir-placement difficult.
Getting the size correct, placing the pump and the tubing optimally, is easier with peno-scrotal.
Also, the infra-pubic is faster, which is good for the surgeon if he wants to squeeze in a lot of surgeries in a short time of course.

I would also consider the fact that the two most reputable surgeons do both use the peno-scrotal method. If they used an inferior method, I don't think they would have gotten where they are.
I am not saying their peno-scrotal method is better, I say maybe, or even likely it is better, but not much speaks for it being worse.

I would forget the focus on the method, and instead find a doctor who you believe will give you the maximum probability of getting a great result.
No infection.
No size loss.
No tubing which is routed wrongly, looks bad and is in the way when fucking.
A great placed pump, behind your balls.
No nerve damage, i.e. no sensitivity loss.

Leave it up to your doctor to choose how to achieve those results.

This was discussed back in year 2017. I made the following post back then, and I will now cut copy paste my post from them, personally I agree with Merrix comments above, and I really wanted penoscrotal. That being said, the below paper states:

I was told that "the most difficult and unpredictable " part of the implant operation involves placing the pump in the proper place in the scrotum. The peno-scrotal approach allows for better control of how and where the pump will "lie" in the scrotum; and , I am told, it also allows for more effective hiding and placing of the tubes (sometimes , I am told, the tubes can be felt during lovemaking in infrapubic installs.)

I just did a quick search on this and there is a paper discussing this topic. the key excerpt is:
".. The advantages of the penoscrotal access reported in this paper are:

– The penoscrotal approach provides an excellent exposure.
– It affords proximal crural exposure if necessary.
– Avoids dorsal nerve injury.
– Permits direct visualization of pump placement.

The disadvantage highlighted for the penoscrotal approach is that the reservoir is blindly placed into the retropubic space, which can be a problem in patients with a history of major pelvic surgery (mainly radical cystectomy). Furthermore this document reinforces the idea that revision surgery is associated with decreased outcomes and may be more challenging with the infrapubic approach.

The ISSM Consensus in Chapter 18 (8), dedicated to penile implants reports that there is no clear advantage of one type of access or other, and the choice depends on the surgeon's preference and that the literature data showed that the incidence of infection is similar between the penoscrotal and infrapubic access. ...."

There is also a statement that:
"...A very interesting study comparing penile prosthesis implant surgery before and after Expert Training with Standardized Operative Technique for residents demonstrated that after the training, penoscrotal access increased from 52.2% to 97.8% ..."

To contradict the above, there apparently was a survey of 42 patients that concluded that both groups were happy, and there was no statistical difference: "....Candela & Hellstrom presented retrospective study comparing satisfaction of the patients submitted to implantation of 3 volume inflatable penile prosthesis with penoscrotal and infrapubic access through a questionnaire sent to 86 patients. Analysis of the 42 questionnaires returned demonstrated no statistical differences in the replies of the two groups in either the factual or perceptual data (9)....."

#4 Is the doctor that I'm currently booked with. Dr. Perito. I have read a lot about him and how busy his office is. How his office is a sausage factory. How he is cocky and arrogant. I've heard about some of his legal issues he had 10 to 15 years ago, etc, etc and it keeps going. However I have watched every youtube video that he has posted. He seems to know Dicks. Either doctor that I pick from out of town is basically going to be a one and done experience. If when I'm back home and I have follow up issues it will have to be with another local doctor. All I'm concerned about is finding the Best Doctor that can Perform Infapubic and perform the surgery well. I don't care if he's arrogant or rude. I will see him twice and fly back home. I'f a doctor is good that is who I want. Some of the best Athletes in the word are the best in the World at their jobs but are arrogant and rude. Enough about Aaron Rogers. LOL


Well, your profile name change had me a bit lost. You're the same person who was going by "Creighill" last month... I see you decided against Perito and chose to go with Kramer. Glad you're during so well. I've really enjoyed following you're story since your surgery.
