….and sorry it has taken so long to make this post. Once the decision had been made and “the dice had been thrown”, I felt like I wanted no more information or discussion until I healed. I wanted to wait before I jumped on here to give my accounting of the experience, for a number of reasons. Dr. Goldstein and his team were amazing !!! and he reached out to me everyday post-op to check in and answer questions . As far as surgery, So much of my experience was identical to what others have posted so I’ll start with information I have not seen which I feel could be helpful. My details are at the bottom of the post.
Background: Pills, Injections, Shock therapy, Blood Plasma Therapy, VED, Traction, Peyronies.
Frozen Vegetables: week1 was painful (like any big surgery recovery). I found frozen vegetables shoved down my pajama pants to be way better than ice packs or any other cooling strategies.
Bathroom: I hate to say it but I’d recommend relieving yourself in the bathtub. It’s virtually impossible to get situated on a toilet with a protruding penis and sore body. I was mopping the floor daily.
Week2 and Week3: I was still sitting around in my pajamas in some discomfort, but no worse incapacitation than a cold or mild illness. I got my 6 week post-op moved up to 4 weeks and had sex at that point. It was great not to have to sneak to the bathroom and mess with needles. I could not “in all honesty”, say I would have been comfortable going to any kind of job before week4. Pajamas, Frozen Peas and Netflix were my world for a month. (This question comes up a lot here, and I’ve seen people write of going out into the world on day3, and others speak of being still laid up in month2)
Results: the most profound thing I noticed when attempting to measure (and determine if I had retained my size) was that the orientation of everything had changed. The penis sits separate and above the testicals and before everything was more integrated. I have seen many pictures of erections that look like both my “before” and “after”, so I would simply say “it’s different”. I haven’t really lost any length, but I have sort of lost girth (from 6 inches down to 5 inches) however, the width is quite uniform and before the peronies gave me kind of an uneven width. If I fully inflate just for the sake of inflating, it has the look and feel of a big curtain rod (and the head looks unnaturally small and flaccid). If I inflate to 2/3 and have sexual arousal, the erection is completely indistinguishable from a normal erection for a man my age (and the head and width are very fleshed out).
Summary: I think this is a good solution when all other avenues have been explored and exhausted; however, it is not something to take lightly. At six months, it has become part of me (like my hair piece) and like I imagine breast implants are for women, but there are also times I am very aware that I have basically “a Piece of PVC pipe” in my pants. I had a lifestyle where I was having sex daily, and injecting way way to much trimix, so this was not to tough a decision (as the trimix eventually stopped working), but if my drive was such, that I had sex 5 to 10 times a month, I would have to say injections might make more sense. That’s just my opinion, and it’s coming from a man in his late 50’s. I also think the guys who immediately opt for the implant have a very valid position, in that they avoid all the craziness of all that other stuff I did….only to have me wind up with an implant in the end... I’ll try to get some pictures up here ASAP. I’d really like to find some “before” versions, if I can find some.
The anesthesia was general endotrachial.
The incision was transverse scrotal.
The device was 20 cm with 1 cm rear tip.
The reservoir was 125 ml and placed in the left space of Retzius.
The pump was the original Titan pump.
A compressive scrotal dressing was applied.
20 ml Exparel was utilized for post-op pain control
The Foley was left to straight drainage.
The patient was transferred to the recovery room in satisfactory condition.

So what you are saying is if you were "only" having sex 5-10 per month injections would be preferable to an implant?
Curious as to what makes you feel that way.

IF the injections still worked well...would you rather be on that? Was that better when it worked?
I'm 52 (also in San Diego) and currently QuadMix still works well for me. In a long term relationship.
So many guys on here say they wish they had done the implant sooner...
Do you wish you got the implant sooner and skipped the injection phase?

I say that injections would be an alternative because the amount of time any of us spends having sex is a small fraction of the time we spend engaged in "this thing called life". If I threw a keg party in my garage every night, I might "trick it out" with beer taps and a bar like in a night club. If I drank 2 beers a week, I would probably by a six pack from the grocery store. Again : this is the advice of someone who is 3/4 the way thru his life, so take it with a grain of salt. Obviously, the overwhelming majority of men with ED simply work around it, and are not on this site, so if someone is committed to solving ED once and for all .......and never again having to worry about it....the implant is the way to go.

That is true but I also think the majority of those endorsements nearly always come with caveats as well. I think the general feeling that men with implants have is that while as wonderful and game changing a thing as it is, it is not totally similar to a natural erection. That having an implant in no way trumps a healthy penis. Once every single resource has been exhausted then and only then is it time to seriously consider an implant. In that regard if you have reached a point in your experience where nothing, and I mean nothing, gives you a satisfactory boner then putting off getting an implant could be considered time wasted.

Question guy, i was implanted by a fellow who learned and worked with Dr Goldstein. I had a great implant surgery and experience. not a second thought about the decision i made.

Dr. Goldstein is a great doctor and did an awesome job. The length was exactly what he said it would be, there is no scar, and the placement of the pump is such that it is invisible to the eye. Over the last decade, he gave me a great deal of guidance and let me come to this implant decision on my own.

My opinion: If I were in the last quarter of my life and I were married and I were having sex only a few times a week, (because of my own sex drive) then yes,..... injections might be a very good solution. If you are young and/or a Casanova, then injections are not the right tool for the job, because the more/longer you use the injections, the less effective they become, and the 5 minutes you need to compress the injection site is an issue too. I was injecting and having sex daily and not really compressing for 5 minutes. The shots became less effective, more scar tissue formed, more miss-fires occurred (as I then sometimes landed in the scar tissue).

If you are in San Diego and decide on the implant, Dr. Goldstein is the way to go. I traveled there via trains, ubers, hotels and professional hospice nurses. Your experience would be infinitely simpler than mine. If your 52 and already on Quadmix, you have likely already exhausted BIMIX and TRIMIX, so injections may not last you the rest of your 50's. On the injection forum, there are men who have used them for many years, and have never had to increase the dosage, but that was not my experience.
As far as wishing I did the implant sooner: I don't know yet. I have not had alot of sex experience with the implant because I intentionally did the surgery when I was in-between jobs and relationships. I recovered over the holidays, and then life (and then the pandemic) got in the way. I will say, that after dealing with ED, the injections were like the gods had given me 3 more years of "extra innings". I went from "humiliated ED guy" to "6 hour erection guy" overnight. In a year from now, I may well come on here and say that that I wished i went right to the implant. In all fairness, I can't compare my "colorful injection sex life" with my current "asexual implant life".
Probably the main thing that kept me trying all the alternatives, was that they were all temporary band-aids, and left open the possibility of some kind of miracle recovery. Once I started messing with plasma shots and shock therapy and the injections that once gave me a "6 hour tire-iron", were now giving me "30 minute semi-erections", I raised the white flag. I'm pushing 60 and I needed to have this ED thing crossed off my "list of life issues".
The main point I had hoped to make, is that pills and injections may (in some cases) be the right tool for the job. As someone who tried everything, i felt I had something to contribute by pointing out that they each have pros and cons. I am very happy with the implant results, and super happy to have this behind me. I also don't think its completely sunk in with me yet, that I'm done with ice pack boxes and needles and sneaking to the bathroom, and Dr. appointments and compounding pharmacies etc....

Thanks for all the advice. Think I will keep on the QuadMx until it stops working...maybe miracle cure in next few years
