Hey everyone, iam pretty close of getting my implant surgery done in germany. Just some paperwork and ill be ready to get my appointment.

Still wondering if for any of the people who had implant already the glans engorgement changed after the surgery.
This is what iam really worried about tbh, cause right now i still do have and it works fine and i would love to keep it.

If you read through a couple years of past posts you will find many men's reports of how their glans (and the rest of their spongiosum tissues) respond after surgery.

As I read them, I conclude that whatever engorgement you had of spongiosum tissue will be the same after surgery as before. As long as nothing bad happened during surgery, like nerve damage that results in loss of sensation, for example. The better and more experienced the surgeon, the less likely this is to happen.

Spongiosum tissue (which fill out the shaft and glans) engorge but do not contribute much to the rigidity of an erection. Corpus cavernosum tissue, which fill out the tunica albuginea engorges, and since it is confined inside the tunica (which is not elastic), give the rigidity of the erection. The implant operation inserts the implant inside the tunica and gives the rigidity without (theoretically) affecting anything outside the tunica. Which includes all the spongiosum tissue in the shaft and glans.

I agree with Lost Sheep post. My gland still engorges post surgery without even pumping the cylinders. The cylinders actually just gives the rigidity to the shaft.

I worked hard on an answer to this question back when I was a newbie bionic in 2017; so, I resurrected my answer, complete with figures, and have cut/copy/pasted it here. In a nut shell, I agree with what others have stated "if your surgery goes without complication, then your glans will be similar in performance to what it was before surgery." personal experience: my natural glans does not work very well (ie, i had very severe ED). Interestingly, trimix gel into urethra does help my glans, and I actually get erections with the trimix gel . The trimix erections are not good enough for vigorous sex---though the erection is good enough for impotent guy "soft fuck " sex if I do not inflate the cylinders.

Here is what I was told wen I asked about the glans bulking injection:

1) it lasts roughly a year
2) it costs something more than 700 dollars
3) it is used for "old men" who have a shriveled up wrinkled glans. Eid said that I do not need it
4) I have been told that it "looks really nice" to have a big knob of a head sitting on the implant


Now, remember, glans erection is different from cavernosum erection. Some guys with ED have no trouble with glans erection (before operation) and some have terrible glans erection (before operation). In general, your glans erection likely will not change (better or worse). That being said, some franktalk guys report "better glans hardness" after implant. Many report no change in glans function. (just for completeness, you should know that there is a rare complication of "cold glans syndrome" which might be associated more with infrapubic incisions.)

You should be aware that the glans is part of the "spongiosum" which is a different blood supply from the cavernosum (the cavernosum is where the cylinders are placed)...thus, an operation on the cavernosum (i.e., penile implant) should, in theory, leave the spongiosum intact.

For me, there was no decrease in glans, but no increase either as far as i can tell to date.

Those who really want to work on the glans hardness, the options are:
1) trimix gel -- might help
2) cock ring (definitely will help--based on my personal experience)
3) viagra --- might help

For me, personally, my erection is "good enough" and the glans for me does swell up pretty darn good just before orgasm. So, I , personally, would prefer to travel light and not go for any of the above three possible glans performance boosters--one of the points of the implant is that it gives you spntaneity , so best to get off the rings and gels and drugs.

I think lack of glans engorgement is the product of nerve damage done during a prostatectomy. That is my story. All nerves removed. My glans was shrunken and cold before the ipp. Still is after IPP.

Yes. I still get the same glans engorgement if I am turned on. If I am actually into sex and before even pumping up I actually get pretty hard.

If i am not into it and i pump up you can feel the tips of the cylinders. If i am into it and my glans engorges you do not feel the tips at all.
18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.

now, as a three year bionic, I have a little more opinion and experience with the following three solutions to glans engorgement (or lack there-of):

Those who really want to work on the glans hardness, the options are:
1) trimix gel

Trimix gel was prescribed to me by my local urologist as an attenpt to improve glans firmness during sex (my wife likes the glans hardness since she has learned that glans is now the only indicator of my natural state of being turned on by her). Interesting, when I use trimix gel, I actually get a natural erection in the glans and in the cavernosum !!! It was fun to have a natural erection on top of my cylinders. Because of my severe ED, the firmness of the erection was not great, and I would likely barely be able to penetrate with the erection. For the women, the experience would be aht they might decribe as a "soft fuck" whereby the man can get in, but the penis never really gets super hard. So, for my wife, who has experienced the joys of my implant, would never put up with that sort of semi-chub erection, and I of course use the pumped up cyliders when having sex. All that being said, though, it was neat to have a natural erection again thanks to the trimix (though, as stated already, the erection is not good enough to be prodigious in bed). That being said, I do use the trimix on occasion when I want my glans to be big during sex, and the best part is that when my glans swells up at orgasm time, the swell is even bigger with the trimix on board. oreover, I do notice natural (though weak) erections happening since the trimix has kick started things a little.

By the way, you all should know that Propecia (finestride) has really messed up any remaining erection capability I had. Having the implant, I figured that I had nothing to lose by taking propecia for hair loss. WRONG, propecia led me to lose any glans erections and decreased my semen volume. The good news is that after stopping propecia and using trimix gel, I am back to where I was 6 months later.

2) cock ring
I have written about this in the past, it helps the glans erection a bunch, but I think the ring looks odd, and women might be a little judgemental since you "need help to keep it up", so I do not use the ring since I do not need it, really (ie, my glans, though rather limited, is good enough)


3) viagra -
I tried this. It was not a significant improvement. And I can't stand the headaches, or in the case of cialis, the backaches. Also, the need to time things is not what being a bionic is all about.

So, in my experience, it is best to "travel light" and discard the trimix gel, discard the cock ring, and discard the viagra. A bionic is plenty good enough to satisfy a women, and fixating on the soft glans is a neurotic activity that is not worth the effort---in my opinion

Must say that this was one of my main concerns before the surgery. One month post-op I'm happy to say that everything is working fine. It's a thrill to wake up with morning wood or really just glans engorgement since I have yet begun to pump to full erection.

Where does one purchase trimix gel and is it available without a prescription ? How is it applied ? I used Muse a long time ago prior to implant and it had a little tool to insert the pellet into the urethra.
