"Distal" means "OUTSIDE" the body. If you got 12.5 distal, you got 12.5 cm outside the body. If you got 1.5cm on top of a 22cm implant, you got 23.5cm total length with 12.5 cm distal leaving 11cm proximal (internal crus).
That's almost 1:1 distal to proximal. The numbers he gave may have been inaccurate (the "Distal" measurement, that is). What this would mean is you have a 4.92" (plus ~0.5" for glans) penile length and a 4.33" crus or internal depth/length.
