Follopw-up to How Much the Pills Help in the General Forum

Although the original query was from member "midlifeED" in the General Discussion Forum, I have posted these experiences in the Injection Forum because it seems more apprpriate.

My experience mirrors that of dtwarren1942 in the General Discussion Forum. I assume that my ED is mainly diabetes (type 2) related (there are also many men of 50+ years with ED who do not have diabetes or other obvious reasons e.g. low testosterone).

I started with mild ED about 15 years ago and 25mg of Viagra completely solved the problem - erections as good as when I was a 20 year old. Gradually, over time, I found that I needed 50mg and at about the 12 year point I found that a constriction ring was needed to get a firm erection which would last at least 30 mins (I used an ordinary O ring available from many hardware stores - very cheap and available in a wide variety of sizes). Over the years my doctor suggested I try Cialis both single dose and daily but neither worked. I also used Levitra on my doctor's advice (supposed to be better for diabetics) for about 2 years. Levitra was quicker acting than Viagra but, after a time, I started to get indigestion about 4 or 5 hours after using Levitra which often lasted about 24 hours. Probably an interaction with my oral diabetic pills. So I returned to using Viagra.

At the beginning of this year, I was finding that even with 100mg of Viagra and a ring I was sometimes not really hard enough for easy penetration and tended to lose most of my erection after about 5 or 10 minutes of intercourse. My doctor referred me to the Erectile Dysfunction Clinic at my local hospital where a VED (vacuum erection device) was suggested, but I had already tried this and, whilst it worked for about 15 minutes it not very spontaneous (you have to stop foreplay to use it) and did not give a natural feeling erection. We also discussed MUSE but but I have read that it is not that effective with many men and so I started on Caverject (Alprostadil) which is the only penile injection therapy available in the UK - We do not have Trimix. After the usual trials, I found that I needed about 20 to 25mcg of Caverject to produce a really firm erection which would last between 40 mins to 60 mins depending on the dose. I have now been using Caverject for 6 months, have no problem with pain either at the time of injection or afterwards and it works really well.

I have found that 50mg of Viagra plus 10mcg of Caverject works just as well as 25mcg of Caverject alone. I have had no problems with erections lasting too long (priaprism). Fortunately, ED associated with diabetes means that the (expensive) Caverject is free under our National Health Service and I am allowed 8 x 20mcg dual chamber syringes each month. I hpoe this information will be of help. Don't be put off by the thought of injections!
80 year old Type2 diabetic, long term ED, pills then injections PGE1. After 4 years they became less effective. Implanted 20/3/2018 by NHS. AMS 700 CX 15+4 cm with MS pump. Never had any significant pain. Still working perfectly and used regularly.

Thank you for this clear detailed comment ....i wish that you gave Muse 1000 with viagra 100 a chance coz i read a clinical trial about it or may be u can alternate

day one ( late at night intercourse ) viarga 50 with alpro injection 10 mcg with cock ring
day two ( midday intercourse ) viagra 100 with muse 1000 with cock ring ( u get the benefit of yesterday alpro...i guess
thats too much theory ....with your past few years experience ....we can improve our sex life

MUSE is seriously expensive and I would only be prescribed (for free) either Caverject or MUSE not both. If I lived in the USA I would certainly use Trimix - trials suggest that is more effective than Alprostadil alone and it is likely that I would need a smaller injection volume (perhaps only 10 units rather than the 50 to 60 units of Caverject I need - one 20mcg dual chamber syringe gives you 50 units for the full 20mcg although you can dial up smaller doses). Although, once mixed in the syringe, you are supposed to throw away any unused Caverject, it can be kept in the fridge and remains effective for quite some time. Incidently, if you dial up the full 20ncg dose on the Caverject syringe and inject, there are about 14 units (0.14ml) remaining in the dead space of the syringe. If you remove the Caverject needle you are left with a small rubber plug at the end of the syringe. You can insert the needle of a 5/16ths 30g single use sterile syringe (or bigger) through this plug and extract most of the remaining 14 units. This represents another 5mcg of Alprostadil otherwise wasted!
