I am in completely same situation as you. 30 years old, live in UK and considering implant in 4-5 years time. I have a gf, we have been together for 1 year and recently I told her about my problem and she was really happy that I shared this with her and she seems to be very supportive. If my dick is temperamental even with using meds like Viagra we just use toys for sex like strap-ons, vibrators and whatnot.
