Hey all, been lurking for a while and getting great info about implants, just have a couple of questions for guys that have/had Peyronies and got implanted.
Of the two, would you pick Dr. Eid or Kramer? I’ve seen all the videos Kramer’s done in regards to Peyronies and he seems to do a great job.
I know Eid is highly skilled as well (I’ve seen the almost 180° curve that he fixed) but I had a video consult with him and he said he wouldn’t incise the plaque at all, just put in the implant. Did he misspeak? I feel like I’ve seen posts by guys that used Eid and he did something to the plaque. I obviously want to have a straight penis, but I’d also love to never feel this stupid scar tissue again.
I’m 34, I have a ~45° curve to the right, and mild ED. Not interested in other surgeries or xiaflex since I will probably end up with an implant anyways.
Eid and Kramer are within driving distance, and they’re high volume, which is why I’d like to choose between them.
Anyone with Peyronies care to chime in? Thanks!

I don't know if Eid practices modeling of PD plaques but it's a common procedure now in many cases of PD where they don't excise the plaque and graft. That process is much more invasive surgery as it requires degloving of which I personally am not a fan, but I'm not in the medical field though so take that with a grain of salt. Most doctors I've heard about in the past year or so choose to implant without fixing the plaque first and then they model the penis which is a nice way of saying that they pump it up to maximum and then bend it HARD against the curve to break the plaque free. I had such a procedure done on my PD plaque and my (USA) nickel sized plaque in the middle right side is no longer pulling my penis up and to the right. It's still there and I can feel it if I look for it, but it sort of free-floats in that one place, still attached to the tunica in one place. It doesn't affect my penis shape, length or girth anymore and all that was done was that the doctor "BROKE" it while I was on the table.

When you say you can’t feel your plaque anymore, did you before surgery? Over the summer I could barely feel my plaque and had the bend, but it seems in the last month or so the plaque has gotten more “pronounced”, but the bend is the same. I’m wondering if it’s more palpable because it’s winter and I’m not exercising outside everyday or because I’ve had a few holiday sweets (or both), but either way, I’d hate to have these flare ups after I get implanted.
So obviously the implant corrects the curve, and you still have your plaque, but did the plaque change in size at all? Even just a little smaller?

My plaque was always palpable when I squeezed my penis a certain way so my fingers were on each side of it. It's literally a round plate of calcified tissue that was attached all the way around, to my tunica albuginea of the right corpus cavernosum, on the dorsal right side. The same plaque is still palpable if I look for it, but it poses no issues when inflating now - where it used to pull my penis up and to the right at what I would guess as 25-30 degrees or so, and used to cause a distinct hourglass effect in that spot, it no longer does. When pumped up I don't detect it at all but when deflated I can still find it but as I said, it seems to almost free-float in that one spot.

I think Dr. Clavell is the best for incision and grafting in the US.

BTW, my Dr. is also Eid. I forgot to ask explicitly about incision but i will make sure to ask in my follow up. Based on my research though, I think Dr. Clavell is extremely strong, specially in cases of severe Peyronie's.
I've been doing non-stop traction and VED to hold shortening at bay. However i still have a lot of girth loss which i'm not sure an implant would restore by itself, my scar is in the dorsal part right by the glans which an implant would have a hard time expanding.

I’ve heard good things about Dr Clavell too, the problem is his location. I’m not prepared to travel in the current pandemic but I can’t stand the thought of waiting any more months to take care of this. I’ve been stable basically since the onset, but the last month I’ve had rapid deterioration. The curve hasn’t increased, but the size of the scar tissue has, occasional pain at the plaque site, shorter flaccid, etc.
VED didn’t help, and restorex aggravated the plaque.
Kramer’s method seems the best: expose the corporal body and incise the plaque, all through the scrotal incision. Yet it also seems like his office isn’t open anymore

Luis, in cases like yours, sometimes they use surgical scissors to "blunt" dilate the corpora into the tip. I've even seen where Kramer (I believe) used the scissors to carefully snip away at the fibrosis to get the tips to seat better.
My doctor had to extend my corporotomy on my right side back into my crus to clean out fibrosis because he couldn't get the rear tip to seat. I'm not sure what method he used there though, scissors, cautery, scalpel?

I had/have Peyronies and have been implanted. My Dr did not do any graphing.
Here is my story...
I had a 90 degree curve straight upward and the hourglass disfiguration right at the bending point of the curvature. If I could get an erection it wasn’t sufficient or hard enough for penetration and the Dr told me not to attempt sex as it could cause more problems. He told me the Xiaflex was expensive, painful, had some serious side effects and it likely would not work. I did not have those injections. I tried everything I read or heard about that might help. Nothing did. As time went along I noticed I was loosing both length and girth. Fast forward a few months and I had the implant surgery. That was in May 2018. According to the Dr, in the operating room they pumped my implant and it was in the neighborhood of a 45 degree or so curve so in immediate improvement. I was told not to touch the implant for six weeks. I followed the Drs orders to the letter. At about nine months there was no visible sign of the curve and I could pump the inplant up and use it as intended. I can find the plaque (I had three) if if feel for it. There seems to be no effect from it and as stated above, it seems free floating. I have gained the majority of my size back. I was warned I would likely loose about 1/2” and that is fairly accurate. I followed the Drs instructions to the letter.
I did not use either Dr you mentioned. I visited with several Drs and went with the one that answered my questions to my satisfaction and I felt comfortable with. I suspect time is not your friend so make your decision and move forward. Best of luck!

Yeah i can see how RestoreX could aggrevate certain types of plaques. Specially because of the clamping. Perhaps you can try a vacuum based traction device? I personally love my vacuum-based totalman rod extender. Yeah, VED hasn't helped much. Recently i've been using my DP-4000 to reach 7hg vacuum. I literally feel the scar tissue expanding and breaking off a bit. Hopefully that'll help more, but regaining girth has been way harder than gaining length.
I've actually gained about an inch in length with 6-8hrs of traction per day. I started with RestoreX, then added totalman rod extender, then graduated to using lghanger @ 6lbs + rod extender + vacuum leg strap.
In my first consultation Dr. Eid told me i'd need a Titan 22-24. In my last in-person consultation with Eid he told me i'd need a custom titan 24-26cm
I'm going to try to go for a 26-28cm estimation next time, lol.
I'm from Los Angeles BTW and had no problems at all traveling during Covid. I just stayed in my hotel the whole time, taking the covid test was a bit of a pain but no big deal.
Honestly, if you're getting an implant why worried about aggravating the scar? That's just my opinion. Get as much length as you can before the operation. I aggravate the hell out of my scar but ultimately my penis is waaay better than it was right after my injury. IMO you *have* to disrupt the scar to remodel it.

Yeah I've seen Kramer uses scissors to open up the corpus cavernosum. But I don't believe my scarring is in the chambers, but rather in the tunica. But I'm not sure TBH. Without inicision and grafting i'd have to rely on the cylinders expanding and breaking up the plaque over time. I kind of doubt the results would be anywhere as good as they would be with grafting.
In your case, i've seen surgeons use this sharp "stick" and literally put their body weight to push down on it. There's no risk of perforating anything so it's safer than cutting upwards.
