Hello All,
1) I hear the #1 complaint of implants is loss of length. I don't know the stats but depending on many factors, it seems to me most people lose anywhere up to 1.5 inches, or nothing. Guessing about 0.75" is the average loss. Does that seem right? About 5" - 5.5" is the average penis size from what I hear.
2) With that in mind, for a satisfactory outcome, what do you think is the minimum size one should have (specifically preop before doing an implant)? For example, if you're 4", it's probably not wise to do an implant and find out you're 3" post-op, right? May be difficult to penetrate/stroke. Whereas someone who is above average size (preop) will be fine if they lose an inch or so.
I think it's useful to hear the group's opinion on this, as doctors have incentive.

Re: Minimum size for a satisfactory outcome with an implant
I think the general opinion on here is that a 3" hard implanted penis would be better than a 4" floppy wet noodle.
Early 30s with ED for years from penis enlargement stretching and jelqing. Implant by Dr Eid on 24 June 2021 with a Titan 24cm with +1cm RTE on one side and -1cm cut off on the other side

Re: Minimum size for a satisfactory outcome with an implant
My opinion:
Any decently experienced and dedicated surgeon should be able to (absent complications such as Peyronie's Disease, necrosis, scar tissues, etc) keep ALL of the length and girth you have immediately pre-op.
This does not mean what you had before E.D. Not what you remember (or THINK you remember). But whatever erection you can get (by whatever means - injection, masturbation, oral medications, suppositories etc with the caveat that the erection you get inside a Vacuum Erection Device-VED- will be significantly larger than any other method because of the extra engorgement and tissue displacement VEDs induce) ON THE DAY BEFORE SURGERY.
That is, there is no reason you should lose any length at all from what you have immediately pre-op.
The surgeon measures your tunica albuginea with a measuring tool. From the incision to the distal tip in the glans. This must be done quite aggressively, so the level of experience and confidence of the surgeon is a major factor in getting this measurement right! Then the tool is inserted proximally, down to the bottom of your crus. The two measurements are added together and the proper size implant is chosen. If the surgeon is not confident of the condition of your tissues or of his own competence, he may choose a shorter implant than could fit. This is also where experience of the surgeon is paramount.
So, choose a surgeon carefully - one how has done a lot of implants with a high proportion of satisfied patients, or who is under the supervision of such a highly experienced surgeon or who has trained extensively under a highly experienced surgeon AND!!! who is dedicated to his patients' welfare and favorable outcome above all other considerations. That last part is part of your responsibility to interview your surgeon thoroughly and establish a good working relationship where your desires are recognized and honored.

Re: Minimum size for a satisfactory outcome with an implant
I think most men don't lose length from an implant. Good surgeons these days try to keep the length and do things, like starting cycling early, that restore some of what may have been lost. Using a VED/traction for a couple months before surgery is a big help. I'm personally not "hung", but I don't think I lost anything at all from the implant surgery. In fact, I gained quite a bit from my atrophied pre-surgery self.
Most of the "loss" I've heard about seems to have come from whatever caused the ED in the first place, such as prostate surgery, peyronies, etc. It is true, however, that some of the men here experienced some loss that can be attributed to the implant. Most, though, seem to have lost length before the implant, and just didn't get it back.
Happy to talk with you on PM or whatever if you want to discuss.
Quincy.

Re: Minimum size for a satisfactory outcome with an implant
I got back all of my size. Took me 2 years with ed and peyronies to go from just under 7 to a bit over 5.
After a year with titan I was pretty close to my pre ed size. Another year with my ams. I have my pre ed dick in all respects.
Find a good doc and fo not worry about it.
Hard dick beats anything.

18 cm plus 1 rte titan installed March 2019. Revision March 2020 by Dr. Andrew Todd, Richmond KY. He replaced the titan with an AMS 700 LGX 18 cm cylinder plus 2 rte for 20 cm total length.
Re: Minimum size for a satisfactory outcome with an implant
I lost at most .5"
My Dr. had me using a vac. device 4 weeks pre surgery and he sized me accordantly.
Pre op engorgement(Vac. everyday 2X) is very imp.to receive correct sizing.
I was a 7.5 and am a 7+ now.
good luck
