I've been lead to belive that Kramer & Karpman charge in the range of $19000 to the lower $20000's range. A lot depends on where the sugery is preformed. Not all hospitals or sugery centers cost the same.
If the doctor doesn't accept the Medicare rate. Then they can charge whatever they want.

Very simple. A doctor is not required to accept Medicare if he doesn't want to work that cheap. My understanding is that its a one time decision. If they don't accept it then they can't come back later & start accepting it. Most will accept it because they need the clients early on. But I have heard of doctors that try to limit how many patients on Medicare that they get.
I'm sure locality & specialty all work into the decision. Lets say a doc was going to specialise in cosmetic surgery that Medicare doesn't pay for. Why would he accept Medicare.
Btw, there is a chance that I'm confusing Medicare with Medicaid. So be sure to do your own research with your doctor.
