I had my penile implant surgery over 1 year ago now (September last year) and there are a couple of things bothering me.

One is the loss of size. I didn't really think about this prior to the surgery because I guess I just assumed that I would come out with the same size as what I went in with. While the difference isn't huge I think the girth is what I'm mostly depressed about. I lost about .75 in length and about half an inch in girth.

Can I expect this to come back at some point with future revisions or do I just learn to accept that I am now 'below average' and get on with it.

The second issue is basically my self esteem. I'm wondering how I can have a normal relationship with a penile implant. Like how do I explain this to a woman and not be rejected? The fact is, I feel like less of a man now and I'm not sure how to proceed from here.

From what you described; you are apparently not undersized. If you are undersized, you will see and feel the floppy glans. Did you notice such a thing? Your pre-ED size may not be regained. Most of us lost a considerable length and girth due to years of ED. A perfectly-sized implant will give you what you came with to the operation room. With regular cycling, some men could get extra length, specially with LGX.
You need to make sure that you are pumping to the max, and you are getting to rock-hard erection. Also a size above 5" is more than enough for sex. It is not sufficient if you are comparing it to your pre-ed size. That kind of comparison will affect you negatively.
Expectations from an IPP should be primarily to get the rigidity back, as LS mentioned as well. That what matters the most, not length or girth. Glans engorgement issue is not related to the implant. VED, Cialis, Viagra, and being aroused frequently, will all help to improve the engorgement. You seem to have a brilliant result. Just go out and enjoy what you got.
I also don't agree with the suggestion of switching to Titan to get more length. Going back under knife should have a valid justification, ie not able to use the implant at all.

The cylinder tips do not go into the glans, they stop just below. If they did go into the glans would that help at all with engorgement? I dunno. The problem is that, because of both of those issues the glans isn’t like, very steady and I don’t know if it’s ok for sex or not. But I’m guessing not, I don’t know. I haven’t bothered to try because I’ve been so depressed about this whole situation.

I’ve tried Cialis but it doesn’t seem to make that much of a difference.

If you think that every girl is gonna laugh at your penis you'll keep being depressed with zero improvements. Surgeons don't fix erections or deep self esteem problems.

Your baseline now is an average sided dick which zero hardness issues. Is it perfect? No. Could it be better? Perhaps. But it is at least "good enough" to enjoy one time sex as well as a relationship. At least. One thing is sure: without trying to put it into use you're life won't improve. I'm not blaming you, just saying that overthinking can fuck your mind.

If they don’t go into the glans that means you need a revision surgery.

That's a bit dire, think. Is it not possible that a man's tunica itself might not extend into the glans? (Variation from one man to another being possible.) The insertion of an implant longer than that which reaches to the furthest extent of the tunica is begging for damage, whether or not the implant/tunica combination reaches into the glans or not.

The tips add support so the glans don’t bend.

First thing you have to do is make the call to schedule a post operation check up preferably with a different surgeon so you can get started with getting your revision surgery.

A wise act. A second opinion by someone familiar with implants and implant surgery and in a position to actually examine the patient (unlike some -like mine- received from the internet from someone who has not examined you)

Using a cock ring helps with engorgement of the glans but only if it is very tight but I guess that then risks damage to the implant so I’m not sure about that.

I’ve heard of people using VED but really, what is that going to do? You surely can’t stretch the cylinders to be longer than they are supposed to be without damaging things.

At this point I’m not sure how to feel. I wasn’t that comfortable with my size before and now I am even smaller than that. It’s now two issues instead of one (size and the fact I don’t have a “normal” penis).

Sonnyjim do you mind telling me where you were treated and what model you were given? I also was treated on NHS and although I’m grateful it was free I was not happy with the care and treatment outcome. Also the model used was a Zephyr which is cheaper than the main brands and does not have the same evidence base. One of the problems with the NHS is you hardly get any time with your consultant and end up with lots of unanswered questions. I ended up paying for extra private appointments for that reason

I'm 6 months out. Not happy with the palpable tubing and the transverse incision scar but I have tried to do the Dr Perito exercise twice everyday. Pumped up to 55 to 60 pumps and I went from 5.5" length to 6" and 5" girth to 5.5". Add in a full strength Cialis, Levitra or Viagra and in the mood it looks like my old cock. The glans engorges and I get about 6.2" in length and almost 5.8 in girth. My wife didn't like it at first. After working the cycling and adding the Levitra it's ringing her bell.

I do like the feeling that I can fuck any woman that is receptive if I wanted too. I could go to the Mustang Ranch and have a ball (I won't cause I'm happily married) but it still feels empowering.
