Corporal Deformity (herniating cylinder)

In December 2021 I had an AMS 700 LGX implant to address ED following a radical prostatectomy to get rid of cancer. As soon as the device was inflated for the first time it became clear that something was radically wrong as even when fully inflated I had lost 60% length and 25% girth of my pre-op size obtained by means of a vacuum pump (which I was using frequently pre-op to prevent muscle atrophy). Such is the reduction in size that sex is extremely difficult and highly frustrating. The pump, valve and cylinders appeared to be working correctly with it taking between 6 & 8 pumps between fully deflated and fully inflated but there was little difference in size between the two. When fully inflated the cylinders are hard and straight with no noticeable bulges or bends but I do experience tenderness where the left tip meets the glans.

My surgeon referred me to a specialist colleague of his in London and after an MRI scan, it was diagnosed that the left cylinder had herniated out of the corporal tube and the lower tips were displaced. This, apparently, is a known but rare complication.

I am aware from this site that Proximal Corporal Deformities are not an unknown complication that arise at varying times after implant but I have not read about it happening immediately post-implant so my questions are; Has anyone out there experienced this problem immediately post-op? What symptoms did you have? Did you have revision surgery and if so were there any complications? Have you had any further complications since revision surgery?

Welcome to the site. I do not know if my problem was/is the same as yours. I had a proximal perforation of my left cylinder. There have been articles and different repairs for this as well as tools developed to prevent this from happening. The tool is a rounded bent rod used to dilate the space for the cylinder. This is because the Crus forks and bends inside our body. And the problem was more common before the tool was developed. A surgeon who uses a straight rod to dilate the Crus has increased chance to perforate the tunica. I had pain with thrusting and a girlfriend who gave aggressive hand jobs driving the perforated cylinder back into my body. And I learned to masturbate pulling away from my body instead of into my body. This does of course happen at the time of the surgery and there are at least 2 types of repairs. One is the sling repair and another is the Sail repair. Many time after this happens and heals the cylinder cannot be placed back inside the Tunica as that area closes off. I was lucky in that it seem the doctor that did my repair was able to get the proximal (RTE) end of the cylinder back in the tunica. And he also used a sling repair. I could follow my cylinders down inside my body under my scrotal sack and the right went deep. The left was easier to follow and stayed closer to the surface. My penis pointed to the left from the base right after surgery. This was due to the right cylinder being supported and having something to push against and the left less so. You can find the articles on the web with a search for proximal perforation and problems with IPP surgery. I will try to attach pictures of my CT scan that showed this. But it is easy to feel and when I finally was able to get insurance to send me to a doctor at a university hospital that doctor brought in a number of students to "feel" the difference in the cylinders. The left cylinder is now deeper and feels like the right. I still have a slight bend from the base to the left and my penis will bend to the left but not to the right from the base when inflated but is better than before the repair. And the pain has improved a great deal.

To add the surgeon that did the repair removed only the left cylinder and performed the sling repair and did not use any new device parts. I think this is mostly to do with my private insurance not wanting to pay for devices.
